Basınç ülseri yoğun bakım ve palyatif bakım merkezlerinde sık karşılaşılan bir sorundur. Tedavi süreci uzun, zor ve büyük ölçüde maliyetlidir. Hasta, hasta yakını ve hekim için aşılması gereken bir sorun olan basınç ülseri tedavisinde yara bakımı önemlidir. Bu çalışmada, konvansiyonel yöntem ve L-prolin ile yapılan tedavilerinin etkinliği karşılaştırılmıştır. Yöntem: Haziran 2018-2019 tarihleri arasında anestezi ve cerrahi yoğun bakım, palyatif bakım ve kronik yara servisinde basınç ülseri tanısı almış, evre I-III yarası olan 96 hastanın verileri incelendi. Hastalar konvansiyonel pansuman (Grup 1, n=47) ve L-prolin topikal tedavisi uygulan (Grup 2, n=49) olarak ayrıldı. Hastaların demografik verileri, yandaş hastalıkları, yara yeri, ebatları, tedavisi için uygulanan yaklaşım, pansuman sayıları, tedavi süreleri, nihai yara durumları karşılaştırıldı. Bulgular: Gruplar arasında yara evresi, yeri, büyüklüğü ve pansuman sayısı açısından fark belirlenmedi (p>0.05). Hastanede yatış günleri karşılaştırıldığında, Grup 2'de anlamlı derecede düşük bulundu (p=0.002). Tam iyileşme açısından gruplar karşılaştırıldığında, Grup 2'de anlamlı derecede tam iyileşme sağladığı görüldü (p=0.017). Yara evresinde gerileme açısından karşılaştırmada, grup 2'de yaralarının evrelerinde gerileme olduğu görüldü (p=0.001). Yara boyutlarında küçülme açısından gruplar karşılaştırıldığında, Grup 2'de yara ebatlarında küçülme olduğu görüldü (p=0.001). Sonuç: L-prolin içeren topikal krem uygulamasının özellikle evre I ve II üzere basınç ülserinde güvenle ve etkili bir şekilde kullanılabileceği görüldü.

Abstract BackgroundImmunoglobulins (Igs) are important components of the innate immune system, and fight pathogens as a part of the first defense line. Newborn dairy calves get maternal antibodies from colostrum. Therefore, contents of immunoglobulins in colostrum and serum of cows are essential traits when estimating potential natural disease resistance of calves. In this study, a genome-wide association study (GWAS) was performed to identify candidate genes that are responsible for the observed genetic variation of immunoglobulins contents in colostrum and blood in Holstein cows.ResultsColostrum, blood and hair follicle samples were collected from the 620 Chinese Holstein cows within 24 hours after calving. The concentration of IgG, IgG1, IgG2, IgA and IgM in both colostrum and serum were detected via ELISA methods, respectively. Using GCTA software, GWASs were performed with 88,934 SNPs genotyped by using Illumina 50K (54,609 SNPs) and GeneSeek 150K (140,668 SNPs) chips in which 50K chip were imputed to 150K SNPs with BEAGLE 3.0.4 software. As a result, 20 and 5 SNPs were detected genome-wide significantly associated with contents of the IgG and IgM in colostrum and serum (P<3.16E–6). In addition, 57, 11 and 10 SNPs were suggestive significantly associated with IgG, IgA and IgM traits (P<6.32E–5). Next, a total of 1,083 functional genes were identified that included or adjacent to these significant SNPs with a distance less than 1 Mb. Functional enrichment analysis showed that these genes were involved in immune related pathways, such as immune response, Fc gamma R-mediated phagocytosis, negative regulation of immunoglobulin secretion, humoral immune response, Fc-epsilon receptor and NF-kappaB signaling pathways. By integrating analysis of the functional enrichment and the known QTL data, we identified 21 candidate genes associated with contents of immunoglobulins in colostrum and serum, including ABR, TIMM22, CRK, MYO1C, RILP, SERPINF2, AKT1, BCL11B, HHIPL1, DYNC1H1, HSP90AA1, TRAF3, KLC1, IL6, PYCARD, ITGAM, TGFB1I1, GUSB, CRCP, RABGEF1 and SBDS.ConclusionsIn this study, we identified 21 candidate genes associated with immunoglobulins level in colostrum and serum in dairy cattle. This founding demonstrated the possibility of increasing immunity through selective breeding and provided an important information for molecular breeding of dairy cattle.

Advances in patient care for pressure injuries (PIs) have reduced the prevalence of PIs in Japan, although not in recent years. Several single-nucleotide polymorphisms (SNPs) have been identified in genes potentially associated with PIs. However, individual variance among PI risks require targeted investigations that may lead to the identification of PI susceptibilities or preventive care options that directly influence PI development pathways. This cross-sectional study examined the association between PIs and SNPs in genes related to tissue tolerance in patients in a long-term care hospital in Japan. A total of 178 participants (130 control, 20 with superficial PI history, and 28 with deep PI history) were enrolled in this study of eight SNPs in hypoxia inducible factor 1 subunit alpha (HIF1A), vascular endothelial growth factor C (VEGFC), heat shock protein 90 alpha family class A member 1 (HSP90AA1), myostatin (MSTN), and vitamin D receptor (VDR). The primary outcome was a history of superficial and deep PIs in the last 6 months. SNPs were examined by real-time polymerase chain reaction, followed by multivariate logistic regression analyses of the associations between the SNPs and PI history. The results showed a significant association between VEGFC rs1485766 and the history of superficial PIs (odds ratio = 2.95; 95% confidence interval = 1.07-8.11; p = 0.04). Stratified analysis using the Braden Scale (≤14) indicated a significant association between HIF1A rs11549465 and deep PIs (p = 0.04).
