Prediction of Future Overt Pulmonary Hypertension by 6-Min Walk Stress Echocardiography in Patients With Connective Tissue Disease

Abstract: The 6MW stress echocardiography noninvasively provides an incremental prognostic value of PH development in CTD. This is a single-center prospective cohort study. Larger multicenter studies are warranted to confirm this result.

“…This includes coronary arterial disease, myocarditis, pericarditis, valvular heart disease, and conduction abnormalities (20). Exercise stress tests have been used to screen patients to identify early cardiac affection, e.g., early stage of pulmonary hypertension (21). This is the reason for the inclusion of exercise tests in the current study.…”

“…Patients at risk of pulmonary arterial hypertension at rest may show abnormal flow-adjusted increase in pulmonary pressures during exercise and are more likely to develop subsequent resting pulmonary hypertension [53, 54]. In patients with established or borderline pulmonary arterial hypertension capable of exercising, the level of exercise-induced increase in systolic pulmonary artery pressure and a reduced right ventricular contractile reserve are associated with a poorer prognosis [55].…”

“…Exercise-induced PH screened by echocardiography can detect early PH in patients with connective tissue disease. 27 In PAH and CTEPH patients, the pressure-flow relationship during exercise predicts survival and correlates with established markers of disease severity and outcome. 28 The future paradigm of early disease detection in high-risk patients should ideally be aimed at detecting disease before a rise in resting PAP.…”
