Введение. Немелкоклеточный рак легкого (НМКРЛ) чаще является аденокарциномой (АК). В ходе лечения пациентов при I–II стадиях проводят резекцию опухоли. У части пациентов после операции развивается рецидив. Цель. Изучить возможность использования до- и послеоперационного уровня концентрации CYFRA 21-1 в сыворотке крови, интенсивность флуоресценции рецептора CXCR1 в гранулоцитах (CXCR1, MFI, гранулоциты); доли лимфоцитов, снабженных CXCR2, в общей популяции лимфоцитов крови (CXCR2, %, лимфоциты), а также их комбинированной модели для предсказания рецидива после резекции опухоли у пациентов с I–II стадиями АК. Материалы и методы. В исследовании участвовало 28 пациентов (19 мужчин и 9 женщин) с впервые диагностированной аденокарциномой легкого I (11) и II (17) стадии. У всех была проведена хирургическая резекция опухоли (объем оперативного вмешательства – R0). Уровень показателей измеряли до начала лечения и через 3 недели, 3 и 6 месяцев после операции соответственно на автоматическом анализаторе (CYFRA 21-1) и проточном цитометре (рецепторы). Результаты. Уровень всех определяемых показателей, будучи повышенным до операции, через 3 недели после операции резко снижается до уровня, ниже порогового. У пациентов с развившимся рецидивом опухоли (по данным наблюдения в течение 1 года) первоначальное падение медиан через 3 недели после операции для CYFRA 21-1, CXCR1, MFI гранулоцитов и результата комбинированной модели было выше порогового значения до операции и увеличивалось через 3 и 6 месяцев после нее. Проведение ROC-анализа, построение графиков Каплана – Майера, моделей пропорциональных рисков Кокса показали, что вероятность рецидива по результатам измерения разницы уровня всех определяемых показателей в период 3 недели – 3 месяца, 3–6 месяцев, 3 недели – 6 месяцев после операции и последующего ее использования для расчета регрессионного уравнения комбинированной модели можно предсказать с диагностической эффективностью (ДЭ) 80,5%, 83,4%, 88,3% соответственно; диагностической чувствительностью (ДЧ) 83,2%, 82,6%, 89,2% и диагностической специфичностью (ДС) – 77,5%, 89,3%, 87,5% при пороговых значениях (ПЗ) 0,047, 0,096 и 0,141 соответственно. Выводы. Предварительные результаты имеют высокую информативность в определении вероятности послеоперационного рецидива у пациентов с аденокарциномой I–II стадий. Наиболее высокие предсказательные параметры у комбинированной модели, расчет которой может быть рекомендован для предсказания рецидива.
Introduction. Non-small cell lung cancer (NSCLC) is often represented as adenocarcinoma (AC). In the treatment of patients in stages I-II, the tumor resection is performed. Some patients have recurrences after surgery. Purpose. To study possibilities of using pre- and postoperative CYFRA 21-1 blood levels, CXCR1 receptor fluorescence intensity in granulocytes (CXCR1, MFI, granulocytes), CXCR2-anchored lymphocytes share in total blood lymphocyte population (CXCR2, %, lymphocytes), and their combined model for predicting recurrence after tumor resection in patients with stages I-II of AC. Materials and methods. The study involved 28 patients (19 men and 9 women) with newly diagnosed lung AC, stage I (11) and stage II (17). All of them underwent surgical resection of the tumor (the volume of surgery – R0). Indicators levels were measured before treatment and in 3 weeks, and in 3 and 6 months after surgery, respectively, using an automated analyzer (for CYFRA 21-1) and a flow cytometer (for receptors). Results. The level of all detectable markers, being elevated before surgery, sharply decreased under the threshold value in 3 weeks after surgery. In patients who developed tumor recurrence (according to 1-year follow-up data), the initial decrease in medians at 3 weeks after surgery for CYFRA 21-1, CXCR1, MFI granulocytes, and the combined model result was above the preoperative threshold values and increased at 3 and 6 months after surgery. ROC-analysis, Kaplan – Meier plotting, and Cox proportional hazards models showed that recurrence probability could be predicted with diagnostic efficiency of 80.5%, 83.4%, and 88.3% respectively, by measuring the difference in CYFRA 21-1, CXCR1, MFI in granulocytes and CXCR2 levels (in %) in blood lymphocytes at 3 weeks to 3 months, 3 – 6 months, and 3 weeks to 6 months after surgery, and its subsequent use for calculating the regression equation of the combined model diagnostic sensitivity of 83.2%, 82.6%, and 89.2%, and diagnostic specificity of 77.5%, 89.3%, and 87.5% at threshold values of 0.047, 0.096, and 0.141, respectively. Conclusions. Preliminary results are highly informative in determining postoperative recurrence probability in patients with stages I–II adenocarcinoma. The combined model has the highest predictive values, and its calculation should be recommended for predicting recurrence.
