Objectives: To summarize significant research contributions published in 2021 in the field of clinical decision support (CDS) systems and select the best papers for the Decision Support section of the International Medical Informatics Association (IMIA) Yearbook. Methods: The authors searched the MEDLINE® database for papers focused on clinical decision support (CDS) systems. From search results, section editors established a list of candidate best papers, which were then peer-reviewed by at least three external reviewers. The IMIA Yearbook editorial committee selected the best papers on the basis of all reviews including the section editors’ evaluation. Results: A total of 337 articles were retrieved from which 13 candidate papers were identified. Finally, from the candidate papers, the top three papers were selected. The first paper introduces an innovative evaluation approach to CDS systems, the second compares six health institutions on how they are measuring CDS alert fatigue and the last one adds new evidence on how CDS can help to reduce unnecessary interventions.

O sangue é um recurso vital para qualquer instituição de saúde, é um desafio global lidar com o aumento da demanda e o desperdício desse recurso. O objetivo desse estudo foi desenvolver e validar um aplicativo que analise a eficiência das solicitações de concentrados de hemácias e direcione condutas hemoterápicas em procedimentos cirúrgicos eletivos. Foi realizada uma pesquisa observacional, descritiva com delineamento transversal e validado um aplicativo para gestão de transfusão de concentrados de hemácias em cirurgias. O aplicativo nomeado como Banco de sangue é composto por três módulos independentes: índice de utilização de concentrado de hemácias, índice de pacientes transfundidos e maximal surgical blood schedule; o primeiro analisa o grau de eficiência do pedido em relação a determinado procedimento, o segundo a conduta hemoterápica e o terceiro a quantidade de concentrados de hemácias a serem reservados, quando for necessário. Dessa forma, através de um método rigoroso de elaboração, foi desenvolvido um aplicativo que objetiva o uso racional do sangue em procedimentos cirúrgicos eletivos.

Strategies to improve outcomes among children and adolescents in hemorrhagic shock have primarily focused on component resuscitation, pharmaceutical coagulation adjuncts, and hemorrhage control techniques. Many of these strategies have been associated with better outcomes in children, but the barriers to their use and the impact of timely use on morbidity and mortality have received little attention. Because transfusion is uncommon in injured children, few studies have identified and described barriers to the processes of using these interventions in bleeding patients, processes that move from the decision to transfuse, to obtaining the necessary blood products and adjuncts, and to delivering them to the patient. In this review, we identify and describe the steps needed to ensure timely blood transfusion and propose practices to minimize barriers in this process. Given the potential impact of time on hemorrhage associated outcomes, ensuring timely intervention may have a similar or greater impact than the interventions themselves.
