Predicting Placenta Accreta Spectrum

Abstract: Objectives-The placenta accreta spectrum (PAS) incidence has risen substantially over the past century, paralleling a rise in cesarean delivery (CD) rates. Prenatal diagnosis of PAS improves maternal outcomes. The Placenta Accreta Index (PAI) is a standardized approach to prenatal diagnosis of PAS incorporating clinical risk and ultrasound (US) findings suggestive of placental invasion. We sought to validate the PAI for prediction of PAS in pregnancies with prior CD.Methods-This work was a retrospective cohort… Show more

“…Predictive models to date suffer from imprecision and unsatisfactory rates of false-negative results (missed diagnoses). 47 It is important to note that the absence of ultrasound findings for PAS does not rule out the diagnosis in patients with previa and clinical risk factors.…”

“…Several ultrasound scoring or staging systems have been proposed to improve the prenatal diagnosis of PAS and the prediction of surgical outcomes [62,63]. These are not yet widely used in clinical practice, and validation studies are ongoing [64 ▪ –66 ▪ ].…”

“…e 3-dimensional (3D) power Doppler has been a widely used tool to improve the PAS diagnosis, much better than 2-dimensional (2D) USG [7]. One of the most commonly used parameters is the placenta accreta index (PAI), a qualitative index that estimates the possibility of PAS using USG [8,9]. However, there are still differences between prenatal ultrasonography, intrasurgical, and histopathological findings.…”
