Advances in “Precision Medicine” initiative, also known as “Personalized Medicine” is an emerging approach for disease treatment and prevention that has already led to innovative discoveries and has created “smart” applications and solutions tailored to a person's or a group of individuals' genetic profile, lifestyle, and environment interaction. Already many physicians as part of patient care routinely prescribe various molecular/genetic and other tests enabling them to select personalized treatments that improve the chances of survival and reduce exposure to adverse effects. This initiative should provide to medical and healthcare professionals with adequate resources and readily available solutions so that the target to specific treatments and care of the illnesses is achieved while at the same time protecting the privacy and safety of the individual is secured as well as the Electronic Health Records (EHRs) and whatever additional data is necessary within the context of Precision Medicine. This study, through a literature review mostly, and some case study analysis, examine whether personalized medicine is delivered to the patient in an “accurate” and “precise” way, as expected. This requires that Health and Human Services and other stakeholders and agencies collaborate to solicit the right input from patients while at the same time can identify and address any educational, practical, legal, and technical issues and providing smart solutions. The lack of proper medical education and advanced infrastructure are still major barriers to the adoption of Precision Medicine, therefore, the role of medical training in Precision Medicine is also examined and analyzed. Specific examples are discussed in an integrated, precise, and data‐driven manner to provide “smart” solutions.

In Ethiopia, healthcare service delivery faces various challenges, specifically in relation to prescribing the right medicine to the right patient at the right time. Thus, patients are exposed to challenges ranging from staying on treatment plans longer than necessary, leaving treatment too early and dying of complications. This paper aims to explore the trends, challenges and opportunities of applying big data analytics (BDA) in precision medicine in other locations around the globe and taking lessons for Ethiopia through a systematic review of 14 peer-reviewed articles from three popular databases. The findings indicate that cancer, epilepsy and systemic diseases altogether are areas currently getting big attention. The challenges are attributed to the nature of health data, failure in collaboration of data sharing, ethical and legal issues, interoperability of systems, poor knowledge skills and culture, and poor infrastructure. In addition to the adoption of modern technologies (new and experimental methods, cloud computing, Internet of Things, social networks, etc.), Ethiopia’s government initiative to promote private technological firms could be an opportunity to use BDA in precision medicine in Ethiopia.

Oxali(IV)Fluors: Fluorescence Responsive Oxaliplatin(IV) Complexes Identify a Hypoxia-Dependent Reduction in Cancer Cells

Platinum(IV) anticancer agents have demonstrated the potential to overcome the limitations associated with the widely used Pt(II) chemotherapeutics, cisplatin, carboplatin, and oxaliplatin. In order to identify therapeutic scenarios where this type of chemotherapy can be applied, an improved understanding on the intracellular reduction of Pt(IV) complexes is needed. Here, we report the synthesis of two fluorescence responsive oxaliplatin(IV)(OxPt) complexes, OxaliRes and OxaliNap. Sodium ascorbate (NaAsc) was shown to reduce each OxPt(IV) complex resulting in increases in their respective fluorescence emission intensities at 585 and 545 nm. The incubation of each OxPt(IV) complex with a colorectal cancer cell line resulted in minimal changes to the respective fluorescence emission intensities. In contrast, the treatment of these cells with NaAsc showed a dosedependent increase in fluorescence emission intensity. With this knowledge in hand, we tested the reducing potential of tumor hypoxia, where an oxygen-dependent bioreduction was observed for each OxPt(IV) complex with <0.1% O 2 providing the greatest fluorescence signal. Clonogenic cell survival assays correlated with these observations demonstrating significant differences in toxicity between hypoxia (<0.1% O 2 ) and normoxia (21% O 2 ). To the best of our knowledge, this is the first report showing carbamatefunctionalized OxPt(IV) complexes as potential hypoxia-activated prodrugs.
