Precipitating causes and outcomes of chronic obstructive pulmonary disease exacerbation at a tertiary care center in northeast Thailand

Abstract: Background: Acute exacerbation of chronic obstructive pulmonary disease (COPD) is a leading cause of hospitalization and economic burden. Frequent exacerbations impair quality of life and effect decline of lung function. Objective: We evaluated characteristics of COPD patients with frequent exacerbations. The precipitating causes, outcomes, hospital stay, and cost of admission were also determined. Methods: The study population included COPD patients admitted because of acute COPD exacerbation at Srinagarind H… Show more

“…Our cohort of AECOPD patients was derived from a single tertiary care setting in the southern part of Thailand. The in-hospital mortality rate of the cohort was approximately 11%, which was similar to other studies [13,25,38], including the cohort from which the DECAF score was developed. As our cohort included both AECOPD patients from ICU and non-ICU settings, the mortality rate was much lower than that of one study (38%), including only patients in the ICU [39].…”

“…Although, the diagnosis of AECOPD in our study was based on clinician diagnosis by discharge summary. The finding of participant characteristics in our cohort described herein represents AECOPD because most patients were older men with a history of smoking similar to the previous report in north-eastern Thailand in 2014 [38]. Third, the MAGENTA model carries a relatively higher number of predictors than other decision tools; hence instead of presenting the model in a traditional perspective, the model was presented as a user-friendly, web-based calculator to enhance clinical applicability at the bedside.…”
