Preauricular approach to infratemporal fossa

Abstract: We present an approach to the skull base that allows access to both the infratemporal fossa and the middle cranial fossa with minimal morbidity. This approach is different from most of the previously described approaches in that it uses a preauricular incision, preserves the facial nerve, and avoids the mastoid bone. It involves dividing the zygomatic arch and displacing it inferiorly, dividing the malar eminence (zygoma) and displacing it anteriorly, and cutting the coronoid process and retracting it superior… Show more

“…Lesions in the infratemporal fossa are rare but are challenging pathologies for physicians (Gates, 1988; Wetmore et al, 1986). Over the last few years, close collaborations between neurosurgeons and otorhinolaryngologists have made it possible to reach and treat various lesions of the infratemporal fossa.…”

“…Various operative approaches have been described for the surgical resection of trigeminal neurinomas [5,16,18,19]. Skull base techniques have recently been utilized [1,3,7,15] to provide for a low and wide exposure and limiting the need for brain retraction.…”

“…9 The preauricular approach to the ITF was developed and modified with these concepts in mind. 10,11 This surgical approach is suitable for the resection of tumors arising in ITF and for intracranial tumors arising in the boundaries of the anterior temporal bone or greater wing of the sphenoid and extending into the ITF. It does not allow, however, the safe resection of any portion of the tympanic bone.…”
