Pre-surgical bronchoscopic treatment for typical endobronchial carcinoids

Abstract: Carcinoids are tumors that originate from diffuse neuroendocrine system cells (APUD cells) and represent 1-2% of all pulmonary tumors. Although surgical resection remains the mainstay of treatment, bronchoscopic radical resection of typical carcinoids in selected cases exhibiting endoluminal growth and small implant base has also been explored. Bronchoscopic removal of endobronchial lesions may also reduce the risk of post-obstructive infections and improve pulmonary function, allowing the patient to undergo s… Show more

“…The clinicodemographic profile of cases and types of bronchoscopic intervention including outcome of treatment has been described in Supplementary Tables and , respectively. [789101112131415161718192021222324252627282930313233343536] Various bronchoscopic treatment options as mentioned in these studies were practiced depending on the physician's discretion, expertise, and available resources. All studies have reported high success rates with minimal complications and good long-term outcome.…”

“…[5673637] Bronchoscopic resection followed by surgical approach has gained attraction for treatment of endobronchial carcinoids with successful outcome as reported previously. [233034] The advantages of this combined approach are recanalization of involved airway, resolution of inflammation, better assessment of the tumor base, allowing more accurate surgery planning with parenchymal sparing, reduction of postobstructive infections, and stabilization of patient in order to undergo surgery in better general condition. Bronchoscopic resection of endoluminal tumor provides larger tissue specimens for more accurate histological diagnosis between typical and atypical carcinoid which is an essential step before deciding for surgery.…”

“…Elderly NSCLC patients have the lowest rates of anti-cancer treatment and the poorest prognosis [17,18]. Although some evidence suggests that in the early stages minimally invasive surgery remains the mainstay of the treatment [19,20,21,22,23], for patients with advanced disease, therapy has been limited for some years to chemotherapy, targeted therapies and radiotherapy. Several factors, including limited physiologic reserve, reduced therapeutic options and increased risks of iatrogenic toxicities as well as age-related comorbidities influence mortality in elderly patients [24].…”

“…Thoracic surgery remains the better curative option for non-small cell lung cancer treatment as chemotherapy is not satisfactory in terms of response rate and survival [ 1 - 6 ]. Despite improvements in diagnostic procedures [ 7 - 13 ], only a few patients are eligible for surgical treatment [ 14 ]. As lung cancer shares common risk factors with other respiratory diseases [ 15 - 19 ] abnormal pulmonary function is a frequent finding.…”
