SUMMARYTapeworms (Cestoda) continue to be an important cause of morbidity in humans worldwide. Diphyllobothriosis, a human disease caused by tapeworms of the genusDiphyllobothrium, is the most important fish-borne zoonosis caused by a cestode parasite. Up to 20 million humans are estimated to be infected worldwide. Besides humans, definitive hosts ofDiphyllobothriuminclude piscivorous birds and mammals, which represent a significant zoonotic reservoir. The second intermediate hosts include both freshwater and marine fish, especially anadromous species such as salmonids. The zoonosis occurs most commonly in countries where the consumption of raw or marinated fish is a frequent practice. Due to the increasing popularity of dishes utilizing uncooked fish, numerous cases of human infections have appeared recently, even in the most developed countries. As many as 14 valid species ofDiphyllobothriumcan cause human diphyllobothriosis, withD. latumandD. nihonkaiensebeing the most important pathogens. In this paper, all taxa from humans reported are reviewed, with brief information on their life history and their current distribution. Data on diagnostics, epidemiology, clinical relevance, and control of the disease are also summarized. The importance of reliable identification of human-infecting species with molecular tools (sequences of mitochondrial genes) as well as the necessity of epidemiological studies aimed at determining the sources of infections are pointed out.

Tapeworms are among the oldest afflictions of humans. They continue, even today, to be an important cause of morbidity and mortality, worldwide. Taenia saginata and Taenia solium infections are still common in many developing countries. Diphyllobothrium latum infections, are decreasing, but this tapeworm, transmitted to humans through the eating of raw salmon, can cause severe anemia. Hymenolepis and Dipylidium infections may occur to children. Hydatid disease is a common cause of morbidity, especially among immigrant groups from endemic areas of the Mediterranean; the liver is the most common site of a hydatid cyst. Although surgery has been the mainstay of therapy, drug therapy and percutaneous aspiration have been used more frequently in recent years. praziquantel and albendazole are the two most useful drugs against various tapeworm infections.

Long Fish Tapeworm in the Intestine: An in situ Observation by Capsule Endoscopy

We present the case of a patient who was referred to our hospital after she reported having passed a long, whitish object per rectum. Accordingly, capsule endoscopy was performed using the PillCam SB video capsule. A tapeworm of the species Diphyllobothrium nihonkaiense was detected; it appeared to be freely floating and unfolded in the jejunum and sometimes tangled or irregularly folded in the ileum. The stretching of the strobila by strong peristalsis in the ileum may have resulted in the separation of the caudal portion of the strobila, which descended into the colon and was eventually passed per rectum.
