Background: The flexible ureterorenoscope (URS) and associated devices have developed rapidly. However, despite its therapeutic benefits, URS may be associated with some complications. To the best of our knowledge, there are no studies discussing the complications of flexURS during the learning curve. Methods: A retrospective review of the records of patients who underwent flexURS from January 2005 to June 2013 was performed. To compare the complications after the introduction of flexURS, patients were divided into four groups based on the surgeon's training experience, that is, based on the number of cases performed by the surgeon. A total of 219 cases underwent flexURS. Groups 1, 2, 3, and 4 included 35, 50, 50, and 84 cases, respectively. The complications were classified using the Clavien system (I-IV). Results: The mean operation time and stone-free rate were significantly different (p < 0.001, p = 0.013, respectively). The total complication rates were 13.6, 10, 8.3, and 3.2%, respectively (p = 0.068). The more the surgeon's experience, the less was the complication rate. Despite our best efforts, the incidence of urosepsis was not reduced (p = 0.902). Conclusions: To reduce severe complications, it is necessary to have performed about 100 cases. Increased surgeon experience tended to decrease the risk of severe complications, but the incidence of urosepsis was not reduced.

DERLEMEGiriş S on yıllarda, teknolojideki gelişmeler ve erişkin ürolojisin-den elde edilen deneyimlere paralel olarak endoskopik girişimler çocuk ürolojisinde de; günlük pratiğin önemli bir kısmını oluşturmaktadır. Endoskopik aletlerin küçülmesi, fiberoptik teknolojindeki ilerlemeler, kamera sistemlerinin ve çözünürlüklerin gelişmesi sayesinde üriner sistemin hemen her noktası endoskopik olarak ulaşılabilir hale gelmiştir. Endoüroloji; vezikoüreteral reflü, posterior üretral valv, şok dalga litotripsinin başarısız ya da uygun olmadığı üreter ve böbrek taşlarının tedavisinde etkin ve güvenilir bir yöntem olarak kabul edilmektedir. Laparoskopik ve robotik cerrahideki ilerlemeler, pediatrik yaş grubunda da; bu tekniklerin açık cerrahideki çeşitliliği sağlayacak şekilde uygulanabileceğini ortaya koymuştur (1-3). Bu derlemede, pediatrik endoskopik üroloji alanındaki gelişmelerin güncel literatür eşliğinde gözden geçirilmesi amaçlanmıştır. SistoüretroskopiCerrahi enstrumanların küçülmesi sayesinde prematür yenidoğanlara bile sistoüretroskopi yapılabilmektedir. Endoskopik girişimler için 3 Fr çalışma kanallı 6-7,5 Fr sistoüretroskop, soğuk bıçak kullanımına uygun 8-8,5 Fr rezektoskop sistemleri kullanılmaktadır. Yine ince semirijit ve bükülebilir cihazlarla "retrograd" veya "anterograd" yollarla üriner sistem endoskopileri yapılabilir hale gelmiştir. Öte yandan prenatal girişim, bükülebilir enstrumanların yardımı ile sayılı merkezlerde ve sınırlı endikasyonlarla da olsa fetus üriner sistemine endoskopik girişimler yapılabilmektedir. Üreterorenoskopi (URS)Çocuklarda URS ile taş tedavisi ilk defa 1988 yılında Ritchey ve Shepherd tarafından uygulanmıştır (4,5). Ancak küçük çaplı orifisler nedeniyle gelişebilen üreteral iskemi, perforasyon, striktür gibi komplikasyonlardan ötürü uzun yıllar tercih edilmemiştir. Özellikle son yıllarda daha küçük boyutlarda endoskopların kullanılabilir hale gelmesi, hatta bükülebilir endoskopların ve lazer teknolojisi ile birlikte kullanımı başarı oranlarını oldukça artırmıştır.URS için günümüzde 4,5 Fr semirijit üreteroskop (Ultra Thin Ureterorenoscope 4,5/6,5 Fr, Richard Wolf GmBH, Knittlingen, Almanya) kullanıma sunulmuştur. Bu enstrü-manın dış çapı en distalde 4,5 Fr ve proksimalde 6,5 Fr kalın-lığındadır (Resim 1). Çalışma kanalından 3,3 Fr kalınlığında aletler kullanılabilmektedir. Sancaktutar ve ark.'ları yaptıkla-rı çalışmada, 4,5 F semirijit üreteroskopun çok küçük yaştaki hastalarda bile üreter tasları için düşük komplikasyon oranı, hızlı üreteral angajman, kısa operasyon süresi sağladığını göstermişlerdir (6). Retrograde İntrarenal Cerrahi (RIRC)Çocuklarda böbrek taşlarının tedavisinde genelde şok dalga tedavisi (SWL) ve perkütan nefrolitotomi (PNL) kullanılmıştır (7,8). Daha hassas ve ince üreteroskopların kullanıma girmesi, lazer teknolojisindeki yenilikler ve erişkin hastalarda kazanılan deneyimler sayesinde çocuk hastalar için de böbrek taşlarının tedavisinde RIRC, alternatif bir yöntem olmuştur. Recent finding and new technologies in nephrolithiasis: a review of the recent literature

This review summarizes recent literature on advances regarding renal and ureteral calculi, with particular focus in areas of recent advances in the overall field of urolithiasis. Clinical management in everyday practice requires a complete understanding of the issues regarding metabolic evaluation and subgrouping of stone-forming patients, diagnostic procedures, effective treatment regime in acute stone colic, medical expulsive therapy, and active stone removal. In this review we focus on new perspectives in managing nephrolitihiasis and discuss recentadvances, including medical expulsive therapy, new technologies, and refinements of classical therapy such as shock wave lithotripsy, give a fundamental modification of nephrolithiasis management. Overall, this field appears to be the most promising, capable of new developments in ureterorenoscopy and percutaneous approaches. Further improvements are expected from robotic-assisted procedures, such as flexible robotics in ureterorenoscopy.
