Abstract. Currently, a search for augmenting antibiotics activity is still crucial due to elevated frequency of detecting carbapenem-resistant Gran-positive bacterial isolates. To resolve this, it might be reasonable to combine carbapenems metal-â-lactamase (MâL) inhibitors. Unfortunately, no MâL inhibitors approved for treatment of carbapenem-resistant infections are currently available. Pathogenic bacteria may survive antibiotic attack, exert tolerance and persistence accompanied with the ongoing infectious process. In connection with this, determining dependence between antimicrobialrelated bactericidal effect and exposure time on microbes at 4, 8, 12 and 24 hours after the onset, a so called time-kill assay, is necessary. A synergy between both agents was noted upon reduced microbial population by ≥ 3 log10. A checkerboard array followed by seeding the microplate well contents onto a dense nutrient medium at various time points were used to assess a synergistic efficacy of carbapenems applied together with clodronic acid against MâL-producing VIMgenotype P. aeruginosa 532/14 clinical isolate obtained from patients with infectious complications (minimal inhibitory concentrations [MIC] for imipenem or meropenem were 512 μg/ml), microbial burden 106 CFU/ml. Optical density was measured at two wavelengths (490 and 630 nm) in ELx800 reader, within 4–24 hour exposure time to determine time of logarithmic growth phase emerging in test culture. It is noteworthy that magnitude of optical density is a difference between two bichromatic measurements resulting in remarkably reduced inaccuracy due to scratches or fingerprints left on the plate. It was found that clodronic acid exhibited a synergic bactericidal effect with carbapenems against a clinically resistant MâL-producing VIM-genotype P. aeruginosa 532/14 strain. Upon that, imipenem-related antimicrobial activity was evident as early as 8 hours after the onset decreasing cell growth down to 1.4 log10 compared to control, whereas 12 hours later it resulted in total inhibition of test strain by decreasing growth of the test strain by 6 log10. Meropenem in combination with clodronic acid showed a more pronounced activity: complete absence of P. aeruginosa 532/14 growth by 8 hours of incubation, growth suppression by 3.2 log10, which reached 6 log10 12–24 hours after the onset. Time-kill assay allows to identify efficient combinations of carbapenems and MâL inhibitors, which is of great importance for increasing therapeutic efficacy of patients with severe purulent-septic complications.

Infectious diseases that are caused by bacteria are an important cause of mortality and morbidity in all regions of the world. Bacterial drug resistance has grown in the last decades, but the rate of discovery of new antibiotics has steadily decreased. Therefore, the search for new effective antibacterial agents has become a top priority. The plant kingdom seems to be a deep well for searching for novel antimicrobial agents. This is due to the many attractive features of plants: they are readily available and cheap, extracts or compounds from plant sources often demonstrate high-level activity against pathogens, and they rarely have severe side effects. The huge variety of plant-derived compounds provides very diverse chemical structures that may supply both the novel mechanisms of antimicrobial action and provide us with new targets within the bacterial cell. In addition, the rapid development of modern biotechnologies opens up the way for obtaining bioactive compounds in environmentally friendly and low-toxic conditions. In this short review, we ask the question: do antibacterial agents derived from plants have a chance to become a panacea against infectious diseases in the “post-antibiotics era”.

La resistencia a los antibióticos se ve favorecida por el abuso y el mal uso de los antimicrobianos en medicina humana y animal. Los aceites esenciales son un gran recurso para hacer frente a esta problemática. Melaleuca armillaris pertenece a la familia Myrtaceae, rica en especies con aceites esenciales. Los extractos de plantas han demostrado actividad antimicrobiana en muchas investigaciones. Combinamos este aceite esencial (AE) con tres antibióticos diferentes para evaluar la interacción farmacodinámica establecida entre ellos frente a Staphylococcus aureus. Los antimicrobianos que utilizamos fueron Cloxacillina (CLOX), Rifaximina (RIF) y Eritromicina (ERI). El AE se obtuvo mediante destilación de arrastre de vapor de agua y su composición se analizó por GC–FID–MS. Los componentes más abundantes en el AE fueron 1.8 cineol (72.3%), limoneno (7.8%) y α-pineno (6%). Trabajamos con aislamientos de S. aureus de tipo salvaje (n = 6) de vacas Holstein portadoras de mastitis subclínica, una cepa de S. aureus (SARM) resistente a la meticilina proporcionada por el INEI-ANLIS "Dr. Carlos G. Malbrán" y S. aureus ATCC 29213 como cepa de referencia control. La concentración inhibitoria mínima (CIM) de CLOX, RIF, ERI, AE y de las combinaciones entre los anitimicrobianos y el AE se determinaron mediante microdilución en caldo a pH 7.4; 6.5 y 5.0. Se utilizó el método de tablero de damas para evaluar la interacción acontecida entre el AE y los antibióticos. Se estableció el índice de Concentración Fraccionaria Inhibitoria (CFI). De aquellas combinaciones que produjeron los valores más bajos de CFI, evaluamos el índice de actividad antibacteriana (E), establecido como la diferencia entre los valores Log10 del número de bacterias viables al inicio (nt0) y al final de la prueba (nt24). Para ello se construyeron curvas de muerte bacteriana para los antibióticos en forma individual, el AE y sus combinaciones a pH 7.4; 6.5 y 5.0 en concentraciones de 0.5, 1, 2, 4 y 8 veces la CIM. Consideramos efecto bacteriostático (E = 0), efecto bactericida (E = -3) y efecto de erradicación virtual de bacterias (E = -4). La CIM del AE a pH 7.4 fue de 25 μL/mL para la cepa control ATCC 29213 y 12.5 μL/mL para todas las cepas y aislamientos tipo salvaje analizadas. Estos valores disminuyeron con la acidez del medio en el orden de 2 y 4 veces. Se demostró una clara actividad sinérgica entre el AE y CLOX, permitiendo reducir la CIM del α-lactámico frente a S. aureus. Esta interacción se vio favorecida por la acidificación del medio, donde concentraciones menores de CLOX lograron un efecto bactericida, cercano a la erradicación virtual, en presencia de pequeñas cantidades de AE. La actividad sinérgica entre CLOX y AE fue particularmente importante frente a la cepa SARM. La RIF presentó una elevada actividad antimicrobiana (0.032 μg/mL) contra S. aureus independientemente de las condiciones de pH. Combinando RIF con AE, encontramos un efecto sinérgico en el que el antibiótico se potenciaba, particularmente a pH 5.0 (la CIM disminuyó 8 veces con 1.6 μL/mL de AE). Una mezcla de 0.004 μg/mL de RIF y 12.5 μL / mL de AE permite a pH 7.4 alcanzar un efecto de erradicación virtual (E = -4) frente a aislamientos de tipo salvaje. Algo similar ocurre con la cepa de referencia. La acidificación de los medios mejora la actividad AE/RIF. Así, concluimos que la RIF es un potente antibiótico contra S. aureus que no requiere el uso de adyuvantes. Sin embargo, estos resultados se presentan como muy promisorios para el tratamiento de infecciones estafilocócicas de difícil resolución, debido a la resistencia antimicrobiana de S. aureus a los antibióticos convencionales, donde las opciones de tratamiento son cada cez más limitadas. En el caso de ERI, la actividad disminuyó en medio ácido (la CIM se incrementó 16 veces), pero cuando se combinó con el AE, se mantuvo el nivel terapéutico del antibiótico frente a aislamientos de S. aureus sensibles. La combinación de AE con ERI redujo la CIM del antibiótico frente a todas las cepas y aislamientos y mostró un efecto sinérgico parcial con valores de CFI entre 0.5 y 1. Para los aislamientos resistentes, a pH 7.4, 25 μL/mL de AE redujeron la CIM de ERI 16 veces y a pH 5.0, 12.5 μL/mL disminuyeron la CIM más de 8 veces. Se obtuvieron resultados similares con los aislamientos salvajes y la cepa de referencia. Los AE pueden actuar como reguladores del metabolismo intermediario activando o bloqueando las reacciones enzimáticas a nivel ribosomal, o incluso cambiando las estructuras de la membrana. Por lo tanto, estos resultados indican el potencial modulador del AE de M. armillaris junto a ERI y pueden ser una alternativa interesante para el tratamiento de enfermedades infecciosas causadas por S. aureus resistente.
