Potential of Direct Agglutination Test Based on Promastigote and Amastigote Antigens for Serodiagnosis of Post-Kala-Azar Dermal Leishmaniasis

Abstract: Post-kala-azar dermal leishmaniasis (PKDL) is a dermal complication, a sequel to kala-azar. Diagnosis of PKDL presents a challenge due to the low parasite burden in the lesions. The direct agglutination test (DAT) based on promastigote and amastigote antigens of Leishmania donovani of indigenous isolates was developed to diagnose PKDL, and the results were compared with those of the rk39 strip test. The sensitivities of DAT for antileishmanial antibody detection, based on promastigote and amastigote antigens a… Show more

“…A comparison of the various diagnostic methods for identifying PKDL cases is reviewed in Table 3. So far, rk39 dipstick test has proven to be useful for PKDL diagnosis, although caution should be maintained to differentiate antibodies due to past VL infection (Salotra et al 2001b;Singh et al 2005). Sensitivity of nested PCR assay is greater than primary PCR allowing reliable diagnosis even in macular cases (Sreenivas et al 2004).…”

“…For detection of polymorphic PKDL (comprising both macular and papulo-nodular skin lesions), the sensitivity of the rK39 strip test is 95.6%, and for macular PKDL, the sensitivity is 86.3%. 6 This report deals with three cases where relying on the rK39 strip test led to misdiagnosis and subsequent therapeutic delay. The study was approved by the Institutional Ethical Committee, and peripheral blood or skin biopsy was collected after obtaining informed consent from the parent/ guardian of the minors.…”

“…4 The K39 epitope is highly conserved in the visceralizing species of Leishmania and was used to develop a diagnostic test for VL and PKDL. 5,6 In cases of VL, the rK39 nitrocellulose-based dipstick test showed high sensitivity and specificity 4 and therefore, has gained substantial popularity for its ease of use, especially in the field setting. For detection of polymorphic PKDL (comprising both macular and papulo-nodular skin lesions), the sensitivity of the rK39 strip test is 95.6%, and for macular PKDL, the sensitivity is 86.3%.…”

“…Histopathological diagnosis of PKDL is not very sensitive or specific, as visualization of intact parasites in tissue sections is difficult. Immunohistochemical staining is complex and has varied degrees of sensitivity (8,9,10).In recent years, several studies have proved that PCR is a very sensitive and specific technique for detection of leishmania DNA (11,12,13,14,15). Few PCR methods have been developed for PKDL diagnosis, but its efficacy on biopsy specimens from various types of lesions has not been assessed properly (12,15).…”
