Potential Interaction Between Warfarin and High Dietary Protein Intake

Abstract: A 55-year-old Caucasian man was receiving warfarin therapy after undergoing aortic valve replacement. His international normalized ratio (INR) was stabilized with warfarin 95 mg/week for 5 weeks. Commencement of a low-carbohydrate, high-protein diet resulted in a series of subtherapeutic INRs that led to a 16% increase in the dosage requirement to maintain therapeutic INRs. After the patient discontinued the diet, his INR increased, and several dosage reductions were required until his INR stabilized with his … Show more

“…Concerns over the interaction between WF and foods have been reported in case reports and reviews (Holbrook et al ., ; Nutescu et al ., ; Paeng et al ., ; Leung et al ., ; Hornsby et al ., ). In these reports, some symptoms induced by ineffectiveness of WF or unexpected prolongation of clotting time including excessive bleeding have been encountered mostly in patients who suffered venous thrombosis, cardiac infarction, or cerebral embolism.…”

“…21 Patients taking warfarin are at particular risk of interactions with dietary supplements, yet approximately 30% use herbal or natural product supplements on a regular basis. 22 There is a possible interaction between warfarin and a highprotein diet.…”

“…Accordingly, considering the high content of bioactive compounds and phytochemicals in plant foods [160], a personalized nutrition (Figure 1) should take into account potential food-drug interactions in veterans who are in treatment for comorbidities. In agreement, dietetic interventions based on personalized dietary advice have also been suggested for old subjects [161], in order to avoid potential food-drug interactions [162][163][164][165][166][167][168][169][170]. In particular, the effect of high-protein, low-carbohydrate diets on warfarin efficacy should be taken into consideration [169,170].…”
