Potential implications of CYP3A4, CYP3A5 and MDR‐1 genetic variants on the efficacy of Lopinavir/Ritonavir (LPV/r) monotherapy in HIV‐1 patients

Abstract: Introduction Several genetic single nucleotide polymorphisms (SNPs) in biotransformation enzymes (CYP3A4, CYP3A5) or transporter proteins (multidrug resistance MDR1 gene product, P-gp) are involved in PI metabolism so that PI pharmacokinetics is characterized by a large inter-individual variability. The aim of this study was: (i) to develop an in-house PCR/direct sequencing, based on DNA purification of full-length CYP3A4 and CYP3A5 genes (SNPs) and MDR1 C3435T variant; (ii) to investigate association of CYP3A… Show more

“…We also saw a modest impact of this variant on lopinavir pharmacokinetics in children. While other polymorphisms in genes encoding for CYP3A4 and ORM1 have been implicated in altering the pharmacokinetics of protease inhibitors including lopinavir [14, 15], we did not find a significant influence in the current study.…”

“…Cytochrome P450 contain isoenzymes which metabolize many drugs in the liver [ 1 ]. To understand this complicated system, CYP2C19 and its genotypes can be used to describe the characteristics of the isoenzymes in cytochrome P450.…”

“…In an interesting case with lopinavir/ritonavir monotherapy in individuals after virologic suppression, individuals with the MDR1 TT variant were significantly less likely to have treatment failures compared to individuals who did not have this polymorphism. [136] Notably, while adherence was the most strongly associated factor with treatment efficacy, being able to identify individuals who do not have this polymorphism may identify subjects who would benefit from TDM and higher lopinavir/ritonavir doses. Having advanced knowledge for cases when therapeutic drug monitoring may be beneficial for patients may lead to better HIV disease outcomes and improved quality of life.…”
