Potential Impact of Modifiable Clinical Variables on Length of Stay After First-Time Cardiac Surgery

Abstract: The STS risk model was reliably predictive of short and extended LOS but did not allow prediction of exact LOS in days. Accounting for potentially modifiable clinical variables, such as low hematocrit and blood transfusion, especially in elective patients, should lead to shorter LOS, higher satisfaction, and reduced financial burden.

“…In line with other reports, over time, patients undergoing CABG in Israel were older, sicker, and have more advanced coronary disease . The length of hospital stay after both procedures were longer compared with those reported by the Society of Thoracic Surgeons and the National Cardiovascular Data Registry . Similar to mortality, it is impossible to reliably compare our outcomes with any benchmark without using validated length‐of‐stay prediction models.…”

“…The majority of patients in Israel are discharged to home, whereas a significant proportion of patients in the United States are discharged to extended-care facilities. 28,29 Parallel to the dramatic change in the volume and type of coronary revascularization procedure, we observed a 46% decline in IHD-related mortality in Israel during the study period-from 84.6 per 100 000 to 46 per 100 000. This absolute rate, as well as the steepness of decline are among the best in the OECD, and substantially better compared with the United States.…”

“…Perhaps resampling could shed some light on this matter.Furthermore, LOS data are generally right-skewed and ordinary least squares regression does not adequately accommodate large LOS values. When used for prediction, least squares regression will produce expected LOS values that follow a bellshaped distribution, in contradiction with the right-skewed observed LOS in Figure 1 of Ad and colleagues [1]. Figure 2 in Ad and colleagues [1] shows that expected LOS values (y-axis) are shorter than the observed LOS (x-axis), fact confirmed by the Pearson correlation of 0.632 (1).…”
