Potential Harmful Effect of Iodinated Intravenous Contrast Medium on the Clinical Course of Mild Acute Pancreatitis

Abstract: Hypothesis: A worse clinical outcome might be expected in patients with acute pancreatitis (AP) who receive intravenous contrast medium for a nondynamic contrast-enhanced computed tomographic (CECT) study early during hospital admission.

“…Plane CT can show the changes in inflammation around the pancreas, but cannot detect necrotizing pancreatitis without the use of enhancement material [17] . However, enhancement material has been reported to aggravate acute pancreatitis [18] and it is hard to employ enhanced CT in patients with renal failure due to severe acute pancreatitis. At present, CT is the only available diagnostic imaging method of acute pancreatitis.…”

“…According to the Atlanta Classification, most pancreatic abscesses arise at least 4 weeks after onset of symptoms 4 , although others diagnose 'pancreatic abscesses' after 1 50,152 , 2 153,154 or 3 86,146,147 weeks. Interestingly, when performing operative necrosectomy Adjacent to the pancreatic bed is a large collection with predominately fluid-like attenuation (white arrows).…”
