Background and Objectives: We aimed to investigate the carriage of colistin-resistant genes among both patients with a history of antibiotic exposure and apparently healthy adults with no recent healthcare contact. Materials and Methods: Stool swabs were collected from healthy people, and specimens were collected at the infection foci from the patients. Eleven primer/probe sets were used to perform the Multiplex Real-Time PCR assay with the QuantiNova Multiplex Probe PCR kit for screening the carriage of colistin-resistant genes (mcr-1 to mcr-10) and 16S rRNA gene as internal control. Results: In total, 86 patients and 96 healthy residents were included. Twenty two patients (25.9%) were positive with at least one colistin-resistance encoding gene. The mcr-1 gene was the most frequent (16.5%), followed by mcr-9, mcr-6, and mcr-4 genes, where the prevalence was 11.8%, 10.6%, and 9.4%, respectively. No patient was positive with mcr-3, mcr-7, and mcr-8 genes. Eight patients (9.4%) were positive with multiple colistin-encoding genes. Twenty-three healthy people (24.0%) were positive with at least one colistin-resistance encoding gene, and the mcr-10 gene was the most frequent (27.0%), followed by the mcr-1, mcr-8, and mcr-9 genes, where the prevalence was 24.3%, 21.6%, and 13.5%, respectively. No person was positive with the mcr-2 and mcr-5 genes. Conclusions: Our findings underscore the urgent need for enhanced surveillance, infection control measures, and stewardship interventions to mitigate the spread of colistin resistance in Vietnam.

Tularemia is a highly dangerous, febrile zoonotic bacterial infection, endemic to many regions of the Northern hemisphere including Eastern Europe. The current war in Ukraine raises the risk for both natural outbreaks of tularemia resulting from destroyed infrastructure, and potential use as a weapon of war or bioterrorism. There has been a substantial history of outbreaks in prior wars in the region, and tularemia has been previously weaponized and stockpiled as an offensive agent. There are current reports of “mouse fever”, where Russian soldiers are affected with a high fever associated with mice in the battlefield. While antibiotic countermeasures to naturally occurring forms of tularemia are available, these may not be appropriate for long-term risks on the battlefield and could contribute to antibiotic resistance. At this time, there are no FDA or EMA approved tularemia vaccines, which are an ideal alternative to antibiotic prophylaxis. Vaccines have advantages in not requiring diagnosis, are generally safe and effective, leading to long-lasting prophylaxis with little impact to the patient and little need for future medical intervention. The potential for engineered forms of the disease for deliberate use may limit the effectiveness of existing countermeasures. The sporadic nature of outbreaks, and likely outcomes following deliberate use raise a number of complexities in a potential emergency response. Means to optimize potential countermeasure assessment, including responsible and effective vaccine candidate testing are discussed.

Honey serves as a medicinal food that is utilized for both preventative care and the treatment of various ailments. Amidst the contemporary challenge of antibiotic resistance, honey emerges as a promising natural antimicrobial solution. The efficacy of honey in therapy hinges on its mechanisms of antimicrobial action. Thus, this study investigated the non-peroxide antibacterial properties of honey sources from a stingless bee species, Meliponula (Axestotrigona) ferruginea, that is commonly managed in Tanzania. The findings reveal that honey from stingless bees exhibits remarkable antibacterial efficacy against both resistant and susceptible bacterial strains. Notably, the studied honey samples retained a substantial portion of their antibacterial potency (89.9 - 98.7%) even after the removal of hydrogen peroxide. Interestingly, the antibacterial activity of honey did not correlate with its total phenolic and flavonoid content, suggesting the influence of specific bioactive compounds rather than overall phytochemical content. Stingless bee honey was most effective against gram-positive bacterial strains, particularly Staphylococcus aureus. These results underscore the therapeutic potential of stingless bee honey for the management of pathogenic bacteria. Future investigations should focus on elucidating the specific bioactive compounds present in stingless bee honey to bolster its clinical applications.
