Postpartum placental CT angiography in normal pregnancies and in those complicated by diabetes mellitus

Abstract: This study demonstrates, that in insulin treated DM pregnancies the fetus as well as the placenta is larger than normal. However, despite a large placenta, a relatively smaller fetoplacental vascular volume supplies the macrosomic fetus. This finding might explain why fetuses from insulin treated DM pregnancies have high vulnerability to intrauterine and intrapartum asphyxia.

“…The clinical value of placental T2* needs to be explored in other clinical cohorts such as pregnancies complicated by diabetes [33][34][35] and late-onset placental dysfunction 36,37 . In these pregnancies, the predictive performance of fetal Doppler flow is particularly limited, and additional methods focusing directly on placental function are highly needed to improve the antenatal identification of placental dysfunction.…”

“…Imaging of the whole human placenta, using a corrosion technique, has also been investigated [13], finding a significantly smaller number of chorionic artery branches [13] and longer venous and shorted arterial vasculature in fetal growth restriction compared to normal placentas [14]. Standard Computed Tomography angiography has been used to investigate the microvasculature of the placenta, finding no difference in macrovascular volume between normal and FGR placenta, despite a reduction in placental size [15].…”
