“…The studies were mainly conducted in Europe and USA. Five studies were randomized controlled trials (7,8,10,11,13), 3 were phase I/II studies (5,14,15), 3 were phase II trials (16)(17)(18) and 3 were retrospective series (19)(20)(21). Three studies (17,18,22) were considered at low risk of bias while 6 studies were considered at high to serious risk of bias (5,7,10,11,15,16).…”

“…While these studies (RAB et al 1976;ZAHARIA et al 1986;BURGERS et al 1988) as will as the recent prospective analysis at the University of Florida (ELLIS et al 1992) demonstrate that pulmonary irradiation can be delivered in the adjuvant setting with acceptable toxicity, the French Study (1988) did find clinically significant decrements in pulmonary function, perhaps because the radiation and chemotherapy were administered concomitantly. None of these studies demonstrates a statistically significant benefit to adjuvant pulmonary irradiation in terms of survival or disease-free survival (prevention of pulmonary metastases).…”

“…Certainly, in the clinical setting, gross disease in osteosarcoma requires a higher radiation dose for local control than does Wilms' tumor (GAITAN-YANGUAS 1981;. BARRETT 1978;BREUR and VAN DER SCHUEREN 1978;ZAHARIA et al 1986;BURGERS et al 1988;French Bone Tumor Study Group 1988). Given the dose limitation in bilateral whole lung irradiation, it is therefore of interest to explore the use of radiation as an adjuvant agent intended to prevent pulmonary metastatic disease (i.e., to treat presumptive micrometastatic disease).…”

“…In most cases, these series evaluate surgical metastasectomy independent of chemotherapy. Radiation therapy for pulmonary metastases has been disappointing [36,37].…”
