Postoperative High-Dose Steroids Do Not Improve Mid-Term Survival With Native Liver in Biliary Atresia

Abstract: In contrast to previous reports, this pilot study shows that high-dose steroid pulses after Kasai procedure are not effective in postoperative adjuvant therapy protocols and should be avoided in upcoming multicenter steroid studies. Therefore, we recommend extended and randomized multicenter studies to pre-evaluate the supposed effectiveness of alternative steroid protocols, by comparing, 6 months after the Kasai procedure, the number of patients with normal bilirubin.

“…Therefore, all patients with evidence of BA lost, at most, 2 days before portoenterostomy was definitely performed. In our series, long-term outcome in terms of diagnostic parameters cannot be evaluated because the same group of patients was included in two studies concerning different treatment modalities (postoperative use of high-dose corticosteroids [27] and minimally invasive access to portoenterostomy, not yet published). This study presents the largest series of ERCP in jaundiced infants so far and demonstrates the advantage of this procedure in preselected patients suspected for BA and scheduled for surgery.…”
