Postoperative evaluation of endovascular parent artery occlusion for the giant extracranial infectious ICA aneurysm: usefulness of transoral carotid ultrasonography (TOCU)

Transoral carotid ultrasonography (TOCU) is a powerful tool for evaluating ICA lesions located from carotid bifurcation to the second cervical vertebra. In recent case reports, contrast-enhanced transoral carotid ultrasonography (CETOCU), which is essentially TOCU performed with perfluorobutane microbubbles as the ultrasonography contrast agent, produces clear intraluminal image of extracranial ICA, which is not possible with conventional carotid artery ultrasonography (CUS). Superb microvascular imaging (SMI) is a new Doppler imaging technique that employs a unique algorithm to minimize motion artifacts by eliminating clutter signals based on analysis of tissue movement. SMI significantly reduces motion artifacts and allows visualization of low-velocity blood flow in vessels. CUS with SMI and contrast enhanced carotid ultrasound are similar in images, and evaluate plaque neovascularization. We performed TOCU with SMI to two healthy persons, in order to examine similarity between CETOCU and TOCU with SMI. Although there were some inadequacies for small lesions, we found that TOCU with SMI can obtain images similar to CETOCU. TOCU with SMI is expected to be applied to clinical use.

A 77-year-old man with chronic kidney disease was presented to our hospital because he developed involuntary movement of bilateral upper limbs and dysarthria during dialysis. On admission, he showed myoclonus on bilateral upper limbs and dysarthria caused by involuntary movement of the tongue. We diagnosed metabolic involuntary movements due to renal failure. Myoclonus on bilateral upper limbs disappeared on the next day, however, involuntary movement of the tongue remained. Transoral motion-mode ultrasonography (TOMU) revealed that the tongue moved with a frequency of 4-5Hz intermittently, and we made diagnosis of lingual myoclonus. TOMU can measure the frequency of involuntary movements of tongue. TOMU is useful for the diagnosis of oral involuntary movement.

Transoral carotid ultrasonography (TOCU) is a powerful tool for evaluating internal carotid artery lesions located from the carotid bifurcation to the second cervical vertebra. In recent case reports, contrast-enhanced transoral carotid ultrasonography (CETOCU) and superb micro-vascular imaging (SMI) which are useful in endovascular therapy are emerging. Lately, three-dimensional (3D) and four-dimensional (4D) ultrasound have been widely used in angiology. We developed 3D-TOCU and used it in a healthy man to confirm the utility of the 3D technique. Mutual conversion to the long axis from the short axis is possible based on an image of a vessel using multiplanar reconstruction (MPR) in 3D-TOCU. By operating the ultrasound machine, a novice examiner can obtain an adequate image in TOCU. TOCU performed with single sweep mode can produce the intraluminal vessel image from proximal to distal on the short axis, which is not possible with conventional TOCU. 3D-TOCU is expected to be clinically useful.
