Postoperative calcitonin study in medullary thyroid carcinoma.

Abstract: Calcitonin (CT) is a sensitive marker for medullary thyroid carcinoma. Normalization of early postoperative CT level is a favorable prognostic factor. The aim of this study was to establish the prognostic value of CT-level kinetics by preoperative tumor stage and postoperative elimination rate. Blood serum from 22 medullary thyroid carcinoma (MTC) patients without remote metastases was assayed. A commercial RIA DSL-1200 test was used to assay CT levels. Basal CT 42 pg/ml and stimulated CT 120 pg/ml were consid… Show more

“…In some patients who are surgically cured, the calcitonin level declines rapidly within the first postoperative hour, achieving an undetectable level with the first few days after the operation. 114116 However, owing to differences in clearance, it has been proposed that 3 months postoperative is the optimal time to determine serum calcitonin levels. 110,114 If calcitonin is undetectable at 3 to 6 months postoperative, it should be measured every 6 months for 1 year and then annually thereafter.…”

“…Serum calcitonin level is the most sensitive and specific tumour marker in the preoperative diagnosis and the post-surgery follow-up [1]. The diagnostic imaging of MTC is still problematic.…”

“…CLT values (basal or stimulated by pentagastrin, calcium, or both) are nearly always elevated in MTC [ 201 ], being a specifi c and a sensitive marker, and elevation after surgery suggests persistent or recurrent disease [ 202 ]. In provocative testing, plasma CLT concentrations are measured before and 2 and 5 min after intravenous administration of the CLT secretagogue.…”
