Postinfarction ventricular septal rupture: Repair by endocardial patch with infarct exclusion

Abstract: A novel operative technique for postinfarction ventricular septal defect has been used in 44 consecutive patients. The operation consists of excluding rather than excising the infarcted septum and ventricular walls. This is accomplished by performance of a left ventriculotomy through the infarcted muscle and securing a glutaraldehyde-fixed bovine pericardium patch to the endocardium of the left ventricle all around the infarcted myocardium. The ventriculotomy is simply closed over the pericardial patch. There … Show more

“…David and Komeda reported the possibility of reconstruction of the left ventricle with a single patch repair. 6,7) This technique further improved rates of surgical mortality, because it involves placing an intracavitary endocardial patch to exclude the infarcted myocardium and maintain the ventricular geometry. From 1996 to 2003, we performed VSP repair in five patients by employing the DavidKomeda infarction exclusion method.…”

“…Surgical repair was performed by slightly modifying technique described by David et al (1995), with the use of endoventricular patch of autologous glutaraldehydefixed pericardium. All patients were operated on through a median sternotomy, cardiopulmonary bypass was accomplished with bicaval venous drainage; cardiac arrest was achieved with cold, antegrade, blood cardioplegia.…”

“…However surgical repair is associated with a high rate of mortality even in more recent reports (David et al, 1995;Deja et al, 2000;Labrousse et al, 2002). The most important predictor of operative mortality is preoperative hemodynamic instability.…”
