Posterior Circulation Occlusions May Be Associated with Distal Emboli During Thrombectomy

Abstract: Background Distal embolization or movement of the thrombus to previously uninvolved vasculature are feared complications during stroke thrombectomy. We looked at associated factors in a consecutive series of patients who underwent thrombectomy with the same endovascular device. Methods We included all patients with acute ischemic stroke in the anterior or posterior circulation, who underwent thrombectomy with the same thrombectomy device for acute stroke from 2013 to 2016. Distal embolization was defined as an… Show more

“…Distal embolization has been reported to be due to clot fragments during thrombectomy procedures and is predictive of worse clinical outcomes (25). We found similar overall rate (4.8%) of distal embolization as previous studies reported (14, 26). But 7.1% of patients in heparinization group developed distal embolization compared with that in the non-heparinization group, which we postulated that the softening effects on the thrombus derived from the antithrombotic effects of heparin may potentially generate clot fragment and cause distal embolization.…”

“…The secondary safety outcome was any ICH on follow-up CT or T2 * MR images and distal embolization during MT. Distal embolization are procedure-related complications that filling defects distal to the occlusion presented on DSA after the retrieval or aspiration, but absent on preceding CTA/MRA or the initial DSA were considered as distal embolization as previous reported (14). The long-term functional outcome was assessed with mRS (range 0~6) at 3-month follow-up by trained research coordinators who were blinded to subjects' baseline characters.…”

“…In contrast, more classical reperfusion of more than 50% of the involved territory (mTICI2b) failed to predict END. Compared to the anterior circulation, EVT for posterior circulation is prone to Willisian collateral failure by distal embolization (21). Thus, aiming for a higher reperfusion grade may be able to prevent END on VBO, and future advances in EVT may also need to focus on higher reperfusion grades, such as distal vessel thrombectomy (22).…”

“…However, this has not been studied extensively. A study of Yeo et al [25] found that IV-tPA was associated with the occurrence of distal emboli on univariable analysis, but this did not reach statistical significance in multivariable analysis in their study.…”
