BackgroundIt is not clear whether socioeconomic status influences health outcomes among older adults through its effect on physical activity. The aim of this study was to assess the effect of sex and neighborhood socio-economic status on the change in health related quality of life (HR-QOL) as a function of physical activity over a three-year period.MethodsThis cohort study included French-speaking community-dwelling older adults recruited in primary care practices in the province of Quebec and participating in the ‘Étude sur la Santé des Ainés’ (ESA)-Services study on the health of the elderly. Primary care practices were recruited through participating general practitioners (GPs) working full-time in the health administrative region. A stratified sample was comprised of various types of primary care practices (family medicine group, local community health services centers, primary care practices with less than 3 GPs, and with at least 3 GPs). In this study sample, 967 participants with scores ≥26 on the Mini Mental State Examination (MMSE) were included and followed for 3 years to study HR-QOL as a function of reported exercise at baseline and follow-up, controlling for study variables. Analyses were also carried out to study the effect of change in reported exercise at follow-up with respect to baseline and categorised as follows: no change, decrease in exercise and increase in exercise. The interaction terms area of residence socio-economic status*exercise and sex*exercise, were tested.ResultsExercise at baseline did not significantly predict HR-QOL at follow-up when adjusting for all other study variables. Exercise at follow-up was cross-sectionally associated with follow-up HR-QOL. Participants reporting never exercising and those reporting a decrease in exercise reported a lower HR-QOL at follow-up, when controlling for all other study variables. There was no interaction between exercise and sex and socioeconomic status.ConclusionsFor healthy ageing, maintaining and increasing physical activity throughout the years is necessary for improved HR-QOL. Past physical activity does not confer protection against future decline of HR-QOL. Future research should focus on potential moderating and mediating psycho-social barriers associated with exercising in older age populations.

Background. The process of ageing influences all dimensions of social life and personal well-being, but the influence of health on different dimensions of quality of life (QoL) among the elderly is rarely examined. Objectives. The aim of the pilot study is to test the feasibility of a comprehensive study design to evaluate general and dental health as well as QoL in a bi-national sample. In addition, this pilot study should allow for the exploration of potential interactions between QoL, socioeconomic, health and oral health variables. Material and Methods. Individuals aged 64 years and older (n = 100) from university dental clinics of the Wroclaw Medical University, Poland (n = 50) and of the University Hospital in Dresden, Germany (n = 50) were examined. The oral health status of participants was assessed by clinical examination. Socio-demographic, environmental and general health status were evaluated during the medical interview. General quality of life (GQoL) was assessed by an overall question with a visual analogue scale (VAS) from -5 (worst) to +5 (best). Health-related quality of life (HRQoL) and oral health-related quality of life (OHRQoL) were measured with the EQ-5D and OHIP-14 questionnaires. Statistical analyses comprised Pearson's c 2 test, Wilcoxon test, linear regression model for statistical analysis and different multivariate linear regression analyses. Results. For the GQoL-VAS-Score the results for QoL measurements were 1.22 ± 2.62 (x -± SD), for EQ-5D-Score 7.45 ± 2.25 (x -± SD), and for OHIP-14-ADD-Score 11.04 ± 13.56 (x -± SD). Differences between Polish and German populations were observed. Conclusions. The study design proved to be feasible for a senior population. The overall GQoL question, EQ-5D and OHIP-14 were regarded as appropriate instruments. Subjective and objective (oral) health measures showed differences between Germany and Poland. For methodological reasons, these differences are not generalizable, but of value for study hypotheses in larger samples (Adv Clin Exp Med 2016, 25, 5, 951-959).

Purpose This study aimed to analyze the effect of Trauma-Focused Cognitive Behavior Therapy (TF-CBT) on the depression level and quality of life of the elderly in an earthquake-affected district of North Lombok Regency, Indonesia. Design/methodology/approach A Randomized Controlled Trial (RCT) with a population of two elderly groups living in an earthquake-affected district was used in this study. The intervention comprised giving TF-CBT. There were three research instruments applied to determine the variables, namely, the Clinician-Administered PTSD Scale (CAPS-5), the Geriatric Depression Scale 15 (GDS 15) and the World Health Organization Quality of Life-BREF (WHOQOL-BREF). All instruments were adopted and translated using back translation to Bahasa Indonesia, which is appropriate at the suggestion of the WHO. To investigate the effect of the intervention, we used hierarchical linear models (HLM) with intent-to-treat analysis. The patent parameter effect was tested using the Wald test (t-test) with a confidence Interval of 95 per cent. Findings The final analysis applied CAPS-5 and showed that there was a decrease in the PTSD of the respondents, which was down to only 8 (17.8 per cent) in the post-test. The same situation happened for the depression variable (2.8 per cent) after it was assessed using GDS-15C. The Quality of life (WHOQoL-BREF) variable was divided into Physical (50.7), Psychological (57.1), Social (53.6) and Environmental (45.7). These components show there to be a significant result in terms of improving the quality of life of the elderly victims of the earthquake. The finding highlights that applying TF-CBT in the elderly population can significantly drop post-traumatic stress disorder and depression level and enhanced quality of life. Social implications The intervention decreased the depression level and improved the quality of life of the elderly as found in the six- week follow-up. Longer training and integration with the structured local wisdom could be necessary to better address the mental health of the elderly affected by the earthquake. Moreover, strengthening the role of the family as the primary caregiver is required to improve the outcome. Originality/value This is the first study that has attempted to use TF-CBT as a method of treatment for the elderly to decrease their depression and to increase quality of life among the Indonesian elderly who have experienced an earthquake. This paper provides knowledge on the effectiveness of TF-CBT that can be used by therapists to treat depression problems suffered by the elderly in a post-disaster area.
