Post-keratoplasty astigmatism management by relaxing incisions: a systematic review

Abstract: Postoperative visual acuity can be limited by post-keratoplasty astigmatism, even with a clear corneal graft. Astigmatism management can be performed by selective suture removal, adjustment of sutures, optical correction, photorefractive procedures, wedge resection, intra-ocular lens implantation, intracorneal ring segments, relaxing incisions with or without compression sutures and repeated keratoplasty. Relaxing incisions can be made in the graft, graft-host interface or host cornea. Despite the unpredictabi… Show more

“…There are various methods of reducing suture-related astigmatism. Topography assisted suture rotation, selective suture removal/replacement, placement of compression sutures can help reduce astigmatism in early post-operative period79. In late post-operative period, ablative procedures or arcuate keratotomies can be used.…”

“…FLAK is known for its effectiveness and safety in decreasing corneal astigmatism after a penetrating keratoplasty (PKP) [ 9 , 18 , 26 , 27 , 28 , 29 , 30 , 31 ]. In many cases, one AK is usually effective in decreasing the amount of astigmatism.…”

“…In many cases, one AK is usually effective in decreasing the amount of astigmatism. However, residual astigmatism after this procedure is common and may be significant, limiting maximal visual rehabilitation [ 8 , 26 , 27 ]. For patients who undergo a PKP or deep anterior lamellar keratoplasty (DALK), the primary goal of FSAK is to allow the patient to wear contact lenses or spectacles [ 7 , 9 , 32 , 33 ].…”

“…While it is a robust tool, capable of reducing large amounts of astigmatism, its predictability is lower compared with other astigmatism‐correcting procedures (Butrus et al 2007). Therefore, newer AK techniques have been introduced in an effort to improve the procedure’s predictability and safety profile (Ho Wang Yin & Hoffart 2017). The advent of femtosecond laser technology and its incorporation into corneal surgery enables the formation of precise stromal incisions whose depth, length, location and shape can be accurately adjusted.…”
