AimThis randomized, double‐blinded, clinical trial evaluated the effect of oral premedication of piroxicam, prednisolone, dexamethasone or placebo on postoperative pain after single‐visit root canal treatment in teeth with symptomatic irreversible pulpitis and symptomatic apical periodontitis.MethodologyThe trial is reported according to the Preferred Reporting Items for RAndomized Trials in Endodontics (PRIRATE) 2020 guidelines. The protocol was registered at the clinical trial registry (India) (CTRI/2019/06/019818). In total, 160 patients, assigned to four groups, received orally either 20 mg piroxicam, 20 mg prednisolone, 4 mg dexamethasone or a placebo 60 min before root canal treatment. Patients recorded their postoperative pain intensity at 6, 12, 24, 48 and 72 h using a 10‐cm visual analogue scale. Intergroup comparison was performed using Kruskal–Wallis tests with post hoc analysis using Dunns test. Incidence of pain was analysed using chi‐square tests. APvalue < 0.05 was considered to be statistically significant. Binary logistic regression was used to determine the odds of postoperative pain, with incidence of pain as the dependent variable and intervention groups, gender, age and type of tooth as independent variables.ResultsIn total, 156 patients were analysed in the trial, since four patients dropped out during follow‐up. All drugs were associated with a significantly lower incidence of postoperative pain compared to the placebo at 6 h (P = 0.009), 12 h (P = 0.003) and 24 h (P = 0.008). Mean intensity of pain was significantly more intense at 6, 12 and 24 h with the use of placebo in comparison to the other three intervention groups (P < 0.05). Intensity of pain was not significantly different between the premedications used (P > 0.05). One patient in the piroxicam group reported gastritis, whereas no adverse effects were recorded in other groups.ConclusionPreoperative oral administration of a single dose of 4 mg dexamethasone, 20 mg piroxicam or 20 mg prednisolone reduced the incidence and severity of postoperative pain following single‐visit root canal treatment compared to a placebo in patients with symptomatic irreversible pulpitis and symptomatic apical periodontitis up to 24 h. The odds of postoperative pain at 24 h for patients premedicated with 4 mg dexamethasone or 20 mg piroxicam or 20 mg prednisolone were 5.3 times, 3.4 times and 2.5 times less compared to the placebo, respectively.

Aim To assess postoperative pain and bacterial reduction following the use of XP‐endo Shaper versus conventional rotary files in preparation of oval canals with necrotic pulps. Methodology This superiority, parallel, randomized, double blinded clinical trial was conducted in the clinic of the Endodontic Department, Faculty of Dentistry, Cairo University, Egypt. Sixty single‐canalled mandibular premolars with necrotic pulps were randomly assigned into two equal groups. Canals were instrumented using XP‐endo Shaper files in the intervention group and iRaCe rotary files in the control group. Bacterial samples were taken before and after canal instrumentation. Incidence and severity of postoperative pain was assessed using a modified VAS after 6, 12, 24 h and daily for 5 days. A culture method was used to assess the number of bacterial colony forming units. Incidence of analgesic intake as well as flare‐ups was recorded. Data were analysed using chi‐square, Fisher Exact, Mann–Whitney, Independent t‐test, and Spearman’s correlation for pain and bacterial counts. Results The XP‐endo Shaper compared with the iRaCe group was associated with a significantly lower incidence of postoperative pain at 6, 12, and 24 h (P = 0.039, 0.047, and 0.026, respectively), and severity of postoperative pain at 6 h (mean difference: 1.33, 95% CI: 0.307–2.352, P = 0.02), 12 h (mean difference: 1.1, 95% CI: 0.26–1.936, P = 0.007), 24 h (mean difference: 0.94, 95% CI: 0.178–1.701, P = 0.008) and 48 h (mean difference: 0.97, 95% CI: 0.192–1.747, P = 0.038). There was a significant decrease in bacterial count following canal instrumentation in both groups (P < 0.001) with no significant difference between them (mean difference: 0.83 × 105, 95% CI: 0.336 × 105–1.996 × 105, P = 0.56). A weak correlation existed between postoperative pain severity and bacterial counts (P = 0.54). There was no significant difference in analgesic intake between the two groups (P = 0.085). Flare‐ups occurred in 3.3% of teeth in the iRaCe group, while no flare‐ups occurred in the XP‐endo Shaper group. Conclusions XP‐endo Shaper was associated with a significantly lower frequency of postoperative pain for up to 24 h, and lower severity of postoperative pain for up to 48 h compared with iRaCe files. Both systems were equally effective in bacterial reduction from oval root canals with necrotic pulps.

This systematic review aimed to evaluate the adherence to the CONSORT Statement in clinical studies comparing different instrumentation techniques in Endodontics. Methodology: A systematic search was carried out through Medline databases. There was no restriction on publication year or idiom. Solely randomized controlled trials (RCTs) were included. The articles were evaluated in compliance with CONSORT. Descriptive analyses of the number of studies by journal, country, and quality assessments were performed with RoB 2.0 Cochrane Collaboration’s tool for assessing risk of bias in RCTs. A total of 6779 studies were identified, and only 33 meet the eligibility criteria. In relation to the overall risk of bias, eight out of 33 were classified as at ‘low’ risk of bias. The overall CONSORT score for the included studies in this review was 22 ± 6.2 points. Significant differences among countries were observed (p = 0.03). The range of years had a significant influence on the average CONSORT score (p = 0.01). The adherence of RCTs of endodontic instrumentation to the CONSORT Statement requires improvements. Adherence to the CONSORT statement will also reduce the high or some concerns risk of bias of studies in the field. CRD42020160552
