Post-burn breast reconstruction using an artificial dermis—a long-term follow-up

Abstract: BackgroundFull thickness burns of the chest in childhood are a devastating problem that requires challenging reconstructive options.Integra is a bilaminate artificial dermis composed of shark chondroitin 6-sulfate and bovine collagen. The dermal matrix serves as a scaffold for fibroblasts and endothelial cells. Vascularization of the matrix begins after 2–3 weeks, and eventually, the matrix incorporates with the tissue to create a new dermis. The main advantage of the Integra is that the neodermis is of the sa… Show more

“…The mainstream application of these dermis substitutes consists of a two‐stage procedure: placement of the bilayer dermal substitutes and split‐thickness autologous skin grafting after 2–4 weeks. Although favourable functional and aesthetic results of such a procedure were reported, some drawbacks should not be ignored, including immobilization of the upper extremities for 2–4 weeks, need of secondary surgery and increased costs due to prolonged hospital stay …”

“…Although favourable functional and aesthetic results of such a procedure were reported, some drawbacks should not be ignored, including immobilization of the upper extremities for 2-4 weeks, need of secondary surgery and increased costs due to prolonged hospital stay. 18,19 Instead, in this study, we used single-layer Pelnac and defatted avulsed skin in a single-stage procedure to manage the degloving injury of the upper extremities with bone/tendon exposure and found favourable functional and aesthetic results. In the previous report, we used a bilayer Pelnac regeneration template in a singlestage procedure for the successful management of 36 cases of traumatic tissue defects and bone/tendon exposure at the finger tip, without the need of a secondary skin graft, 20 but considering the size, location and severity of the wound, the surgical procedure was not indicated in the cases of the present study.…”

“…This is because direct closure is much less expensive when compared with biologic wound agentassisted secondary intention wound healing and does not require multiple weeks of dedicated wound care. 20 Large mucosal defects may be reconstructed with locoregional tissue transfer or by secondary intention wound healing. In regard to locoregional tissue transfer, smaller defects may be reconstructed by advancing the buccal mucosa using a multitude of flap designs to repair the defect.…”
