Abstract: DSA allows more confident detection of pulmonary embolus than does CFA, with no loss in diagnostic accuracy.

“…The imaging techniques most commonly used to diagnose PE are the nuclear medicine ventilation‐perfusion (VP) scan (13–19) via single photon‐emission computed tomography (VP‐SPECT), computed tomography (CT) (13–15, 18, 20–25), magnetic resonance angiography (MRA) (15, 20, 24, 26, 27), and the gold standard, conventional pulmonary angiography (PA) (1, 14–17, 19–22, 24, 28–31).…”

“…Pulmonary diseases frequently disturb the normal ventilation/perfusion ratio (V A /Q) that exists between pulmonary alveolar ventilation (V A ) and pulmonary perfusion (Q). Pulmonary embolism (PE) is a common, elusive, and deadly condition, with an annual incidence projected at over 600000 cases (1) and 200000 deaths in the United States alone (2). Regional V A /Q inequality is one of the chief results of PE (3) (i.e., a “mismatch” between regional V A and regional Q, causing regional V A /Q to deviate from its ideal value of 0.8–1.2).…”

“…In mehreren Studien konnte für die konventionelle Übersichtsangiographie festgestellt werden, dass keine relevanten Unterschiede zwischen der digitalen und der Blattfilm-Pulmonalisangiographie hinsichtlich der Diagnosefindung einer akuten Lungenembolie bestehen (Smith et al, 2002;Johnson et al, 1998;Hagspiel et al, 1998;van Beek et al, 1996;vgl. 1.3.1 (Ferris et al, 1984).…”

“…The clinical validity was assessed in several studies [36,[39][40][41], which performed clinical followup, and revealed a sensitivity in the region of 98% and a specificity of 94-98%. However, one should be aware of the fact that the reliability of pulmonary angiography decreases with diminishing calibre of the vessels, i.e.…”
