Positive margins after mastectomy in patients with invasive lobular carcinoma of the breast: Incidence and management strategies

Abstract: Background: Surgical treatment of invasive lobular carcinoma (ILC) is challenging due to its diffuse growth pattern, and the positive margin rate after mastectomy is poorly described. Methods: We retrospectively determined the positive margin rate in those with stage I-III ILC undergoing mastectomy. We evaluated the relationship between management strategy and recurrence free survival (RFS). Results: In 357 patients, the positive margin rate was 10.6% overall and 18.7% in those with T3 tumors. Having a positiv… Show more

“…Accordingly, while the cancer stage was found to be a significant factor affecting surgical margin positivity, it was not the case for the pleomorphic variant. 25 In our study, it is noteworthy that surgical margin positivity was significantly higher in the pleomorphic group than in the lobular group (19% vs 7%; P = .004). In addition, although both groups yielded similar rates of breast conservation surgery and mastectomy, the rate of completion mastectomy was higher in the pleomorphic group (2% vs 15%; P = .001).…”

“…Our findings are consistent with the literature in regard to the higher risk of positive margins in patient undergoing lumpectomy [ 6 ]. Interestingly, Hewitt, et al reported that in patients with large invasive lobular carcinoma (ILC) tumors, mastectomy fails to obtain clear margins [ 30 ]. Out of the 23 patients with positive margins after mastectomy (simple/radical) in our cohort, 15 of them had invasive ductal carcinoma (IDC) and 7 had ILC.…”

“…Additional surgical procedures in ILC are reported in the literature at a rate of 23.1%–44.5% [ 2 , 13 ]. In their study, Hewit et al reported that 10.6% of ILC patients who underwent total mastectomy had positive surgical margins and that surgical margin positivity was associated with large tumor size and short recurrence-free survival [ 14 ].…”
