Background: Central venous access systems are frequently used for delivery of medications; however, few studies have compared surgical and postoperative complications of right versus left access via the subclavian vein (SCV). The aim of this study was to compare the surgical and postoperative complications associated with Port-A-Cath system insertion via the right and left SCV. Methods: The medical records of patients who received Port-A-Cath insertion via the SCV for parenteral chemotherapy between August 2004 and July 2008 were reviewed. The incidence of surgical and postoperative complications was compared between patients who received right- versus left-SCV Port-A-Cath insertion. Results: A total of 1,848 patients were included in the study. Right-SCV catheterization was attempted in 1,029 (55.7%) patients and was successful in 866 (84.2%). Left-SCV catheterization was attempted in 819 (44.3%) patients and was successful in 651 (79.5%). The mean length of postoperative follow-up was 417.3 ± 401.3 and 396.7 ± 379.9 days for the right- and left-SCV groups, respectively. The incidence of SCV puncture failure was significantly lower in the right-SCV group (12.3%) compared with the left-SCV group (16.8%, p = 0.006). The incidence of catheter knotting at the ipsilateral brachiocephalic vein was also significantly lower in the right-SCV group (0.0%) compared with the left-SCV group (0.5%, p = 0.038), as was the incidence of catheter occlusion (1.0% for right SCV vs. 3.5% for left SCV, p = 0.001). Conclusion: These findings suggest that the right-SCV approach is superior to the left-SCV approach for Port-A-Cath insertion.

Wszczepialne systemy dostępu naczyniowego (totally implantable venous access devices -TIVDs) są bardzo dogodnymi, długotrwałymi dostępami do centralnego naczynia żylnego. Stanowią one niezastąpione rozwiązanie u pacjentów z trudnościami w dostępie do żył obwodowych wymagającymi chemioterapii, żywienia parenteralnego, antybiotykoterapii czy częstych pobrań krwi do badań laboratoryjnych. Porty naczyniowe implantowane są przez lekarzy różnych specjalności: chirurgów, anestezjologów, onkologów i radiologów inwazyjnych. Sam sposób kaniulacji naczynia również może być różny: drogą wenesekcji bądź przezskórnej punkcji. W celu osiągnięcia dostępu do żył centralnych najczęściej używane są prawe żyły: szyjna wewnętrzna i podobojczykowa. Implantacja portów naczyniowych, jak każda interwencja zabiegowa, wiąże się z możliwością wystąpienia powikłań ostrych, wczesnych i późnych. Powikłania ostre związane są bezpośrednio z zabiegiem i są wynikiem nieprawidłowej techniki implantacji bądź anomalii anatomicznych. Powikłania wczesne występują w niedługim okresie po implantacji portu. Spośród powikłań późnych w praktyce onkologicznej szczególnie uciążliwe są dwa: infekcje i zakrzepica związana z długotrwałą obecnością cewnika. Mimo wszystkich swoich ograniczeń i powikłań związanych ze stosowaniem portów naczyniowych od ponad trzech dekad nie znalazły one alternatywy. Stały postęp technologiczny i edukacja personelu medycznego pozwala na zastosowanie wszczepialnych systemów dostępu naczyniowego u wielu chorych, co przekłada się na usprawnienie terapii i poprawę jakości życia pacjentów onkologicznych.

Initial experiences with a new design for a preattached intravenous port device

All available conventional ports share a common design, including catheter, locking nut, and port body, and all share two sites of structural weakness. One site is the junction between the locking nut and the proximal end of the catheter. The other site is the catheter fixation site between the locking nut and the protruding stud of the connecting tube. To overcome these shortcomings, we designed a new type of intravenous port which combines the connecting tube into one piece. The aims of this study were to test the mechanical characteristics of the new design and assess its safety in animal study. The prototype of the preattached port was manufactured from biocompatible materials, including PEEK, silicone and polyurethane. All components were assembled with biocompatible glue and mechanical and safety tests were performed to determine the mechanical strength, and tissue reaction of surrounding soft tissue and entry vessels. The mechanical tests showed the new design would not lead to catheter fracture in the oscillation test. The traction test showed significant peak load (18.75 ± 3.29 vs. 26.61 ± 1.75 N; p = 0.036) because of the difference in catheter extension capacity (26.57 ± 4.28 vs. 47.93 ± 2.45 mm; p = 0.012). Significantly smaller endurable injection pressure was identified in the prototype. (90 ± 20 vs. 177.5± 9.48 psi; p = 0.01) The safety test showed good tolerance in beagle dogs and led to no intravascular thrombus and minimal reaction in surrounding tissues. The new prototype preattached port showed good mechanical strength and overcame two potential structural weakness points. The integrated fixation design not only reduced the dimensions of the port device but also provided a greater injection area compared to current designs. It did not cause intravascular thrombosis and produced minimal tissue reaction in surrounding soft tissue, as identified by autopsy. The new design of the fixation device could serve as the basis for the next generation of intravenous ports. © 2017 Wiley Periodicals, Inc.
