Popliteal Artery Pseudoaneurysm Associated with Osteochondroma

Abstract: Osteochondroma is the most common benign bone tumor, which can sometimes cause vascular complications. Here we report two rare cases (a 48-year-old woman and a 28-yearold woman) presenting with pain and a pulsatile mass in the popliteal region. Computed tomography revealed pseudoaneurysm in the popliteal artery, which was closely associated with a protrusion of a femoral osteochondroma. Surgical repairs were performed, and the patients remained asymptomatic during follow-up. Therefore, considering the potentia… Show more

“…A total of 87 arterial aneurysms in patients presenting osteochondroma were identified from 1963 to 2021. Patients were predominantly male (70.1%), with a mean age of 21.8 ± 11.5 years but ranging from 8 4 to 48 5 years old, and we described the case of the youngest patient in the literature having a pseudoaneurysm in osteochondroma, according to our results.…”

“…A regular postoperative course with neither complications nor recurrence was described in all cases during follow-up, which was reported to be up to five years. 5 Preventive treatment of asymptomatic forms is discussed. 32 Vascular examination and monitoring by Doppler ultrasound have been mentioned by some authors, but most authors recommend preventive removal of a solitary exostosis when adjacent to neurovascular structures, especially in the femoro-popliteal junction.…”

“…The constant pressure and pulsatile friction within the popliteal artery cause damage to the arterial wall. 2 The incidence of pseudoaneurysms has been reported as 0.6%–4.8%, developing primarily from access-related evaluations including catheter-based procedures. The reported incidence is varied based on the method of evaluation (clinical vs routine ultrasonography).…”

“…Osteochondromas are typically benign and have been known to undergo spontaneous shrinkage [ 7 ], although the benign nature is not always the case as osteochondromas can undergo a malignant transformation in up to 1% of solitary lesions and up to 3% in patients with multiple lesions [ 1 ]. When left alone, osteochondromas can grow large enough to impinge on important vessels leading to vascular complications [ 8 , 9 ]. These complications demonstrate the importance of recognizing osteochondromas in clinically silent patients and those with atypical locations.…”
