Resistance to anticancer drugs is often mediated by the overexpression of P-glycoprotein encoded by the multi-drug resistance (MDR1) gene. The nuclear receptor, steroid and xenobiotic receptor (SXR), is one of the key transcriptional regulators of MDR1 gene expression. A variety of xenobiotics bind to SXR, and stimulate transcription on xenobiotic-response elements (XREs), located in the MDR1 gene promoter. Diethylhexyl phthalate (DEHP) is widely used as a plasticizer for polyvinyl chloride (PVC) medical devices. Previous studies have shown that a significant amount of DEHP leaches from PVC infusion bags and lines during interventions, such as total parenteral nutrition, blood transfusion, and cancer chemotherapy. Thus, the leaching of DEHP during parenteral chemotherapy for cancer patients may facilitate MDR1 expression in various tissues, including cancer cells, which may promote drug resistance. To examine such a hypothesis, the effect of DEHP on SXR-mediated transcription of the MDR1 gene was studied in the human colon adenocarcinoma-derived cell line, LS174T cells, which endogenously express SXR. DEHP increased the SXR-mediated transcription of the MDR1 gene in luciferase-reporter assays. The induction by DEHP was abrogated when a reporter plasmid containing mutated DRC4 motif in the XRE was used. In a mammalian two-hybrid assay, DEHP recruited steroid receptor co-activator-1 to the ligand-binding domain of SXR. Finally, using real-time reverse transcriptase-PCR, we showed that DEHP increased MDR1 gene expression in a dose-dependent manner. We conclude that DEHP is an inducer of the MDR1 gene in this cell line. As such, the leaching of DEHP from the PVC medical devices may influence the MDR1 expression, which may induce resistance to drugs in certain populations of cancer cells.

Світовий випуск по-лівінілхлориду становить 16,5 % від загального ви-пуску пластмас -третє місце у світовому табелі про ранги полімерних матеріалів. Особливі властивості ПВХ стали причиною широкого використання цієї сполуки у медицині [6,7].Основні властивості. Полівінілхлорид -синтетич-ний термопластичний полярний полімер. Продукт полімеризації вінілхлориду. Тверда речовина білого кольору. Випускається у вигляді капілярно-порис-того порошку з розміром частинок 100-200 мкм, одержуваного полімеризацією вінілхлориду в масі, суспензії або емульсії. Полівінілхлорид стійкий до дії вологи, кислот, лугів, розчинів солей, промисло-вих газів (наприклад, NO 2 , Cl 2 ), бензину, гасу, жирів, спиртів тощо. Обмежено розчинний в бензолі, аце-тоні. Розчинний у дихлоретані, нітробензолі. Фізіо-логічно нешкідливий. Сам по собі полівінілхлорид є досить крихким і жорстким матеріалом. З метою надання виробам з ПВХ пластичності та гнучкості до його складу додаються пластифікатори. З най-поширеніших -це діетилгексилфталат. DEHP має високу сумісність з ПВХ смолами. Залежно від гнуч-кості, необхідної в продукті ПВХ, медичні пристрої можуть містити від 29 до 81 % DEHP [2].Існує декілька причин, завдяки чому ПВХ є на-стільки популярним матеріалом у медицині. По-перше, ПВХ -домінуючий матеріал, що викорис-товується для зберігання рідин, діалізних розчинів, а також крові та її продуктів. Вироби з ПВХ можуть бути стерилізовані паром, витримувати температури від -40 °C і вище 121 °C. ПВХ також можна стерилі-зувати з використанням ПФС (етиленоксид) і з гам-ма-випромінюванням пучка електронів. Основні позитивні властивості ПВХБезпека. Перед тим як ПВХ може бути викорис-таний як матеріал, що застосовується у медицині, він повинен бути ретельно перевірений з токсико-логічної точки зору. Досвід на ґрунті всіх наявних знань з боку міжнародних органів охорони навко-лишнього середовища та охорони здоров'я показує, що ПВХ (без додатків) є безпечним.Хімічна стабільність. Матеріали, що використо-вуються в медичних цілях, не повинні вступати в будь-які хімічні реакції з речовинами (медикаменти, розчини, препарати крові), які зберігаються у кон-тейнерах з ПВХ або транспортуються по магістралям (внутрішньовенні системи, магістралі для екстра-корпоральної детоксикації, ЕКМО тощо). Вироби з ПВХ довели свою ефективність в цій отраслі.Прозорість виробів із ПХВ. Продукти з ПВХ мо-жуть забезпечувати чудову прозорість, яка дозволяє здійснювати безперервний контроль потоку рідини.Біосумісність. Коли пластмаси використовуються в безпосередньому контакті з тканинами або кров'ю пацієнта, необхідна висока ступінь сумісності між організмом і матеріалом. The Scientific Committee on Emerging and Newly Identified Health Risks (SCENIHR) has evaluated the exposure to DEHP for the general population and patients during medical procedures. In many cases the exposure is significant and exceeds the toxic doses observed in animal studies. There is limited evidence indicating a relation between DEHP exposures and certain effects in humans. However, it is recognised that especially the potentially high exposure during medical treatments raise a concern, even in the absence of clinical or epidemiological evidence, for harmful effects in humans. For certain uses of DEHP alternative plasticizers for PVC are available. The Committee got access to toxicity data for eight possible alternative plasticizers and compared their toxicity with that of DEHP. In respect to reproductive toxicity in animal studies DEHP induces more severe effects compared with some of the alternatives. A risk assessment of these available alternative plasticizers could not be performed due to a lack of exposure data from medical devices. Each alternative to DEHP, however, must also be evaluated with regard to their functionality in respect to medical devices. The risk and benefits of using alternative plasticizers should be evaluated case by case.
