Polypoid Spitz naevus: the benign counterpart of polypoid malignant melanoma

Abstract: Polypoid malignant melanoma is a peculiar morphological variant of melanoma with a distinct exophytic pattern of growth. This form of melanoma is usually very thick and the prognosis is accordingly poor. We present here a previously undescribed form of Spitz naevus which had a similar polypoid exophytic silhouette and marked cytological atypia. Despite these close morphological similarities, polypoid Spitz naevus evolves in a completely benign manner. Morphologically, polypoid Spitz naevus can be distinguished… Show more

“…The histomorphology resembles to some extent a malignant melanoma, but the clinical course is benign. In many cases, a halo phenomenon has also been observed surrounding the lesion 1 6. SN is described as particularly common in young adults and children, but a recent published study showed that approximately two-thirds of patients with SN were over 20 years of age 7.…”

“…Differential diagnosis from polypoid Spitz nevus is one of the main concerns regarding pathologic examination because the latter, clinically, reproduces the silhouette of melanoma; also, histologically, it is equipped with cells showing prominent atypia with big nuclei and expansive nodules. 8 Pathologic distinctive features are shown in Table 2.…”

“…7,8 The typical polypoid melanoma is 2 cm in diameter, Clark level IV, ulcerated, and pigmented with high rate metastasis and poor prognosis. 3,4 Clinically differential diagnosis includes neurofibroma, seborrheic keratosis, pyogenic granuloma, squamous cell carcinoma, basal cell carcinoma, and hemangioma.…”
