Purpose: This study identified the relationships between health literacy, self-efficacy, and medication adherence among older people with polypharmacy and analyzed the factors affecting medication adherence. Methods: This was a cross-sectional study using a survey of 95 participants who were on five or more drugs, 65 years or older, and visited one community center in Seoul between August 5, 2019 and August 7, 2019. Data were collected using a questionnaire and analyzed with descriptive statistics, t-test, ANOVA, Tukey test, Pearson's correlation coefficient, and multiple regression via the IBM SPSS 23.0 program. Results: The participants took on average 8.32±3.90 drugs daily. Their health literacy was very low at 3.53±2.07. Self-efficacy and medication adherence were high at 33.63±5.36 and 15.03±3.27, respectively. Health literacy differed based on age and education levels, whereas self-efficacy differed depending on the frequency of medication. No demographic or medication-related factors caused a difference in medication adherence. Health literacy did not correlate with self-efficacy and medication adherence. Self-efficacy showed a negative correlation with medication adherence (r=-.52, p<.001). However, as a lower medication adherence score indicates greater adherence, a negative correlation means higher self-efficacy is linked with better adherence. Self-efficacy influenced medication adherence (β=-0.52, p<.001) with an explanatory power of 27.0% (F=17.01, p<.001) in this group. Also, the negative regression coefficient means higher self-efficacy was associated with better adherence. Conclusion: Medication adherence was generally high, with self-efficacy being the main influencing factor. Hence, intervention programs that promote self-efficacy should be implemented for older people with polypharmacy.

Purpose: This study aimed to construct a model to predict whether the elderly population over 65 years of age will experience falls according to multi-drug therapy and related factors. Methods: Decision tree analysis was used to analyze data from the 2020 national survey of the living condition and welfare needs of older Koreans. Results: The fall experience rate was 23.7%, which was the highest in the case of diet management due to disease while taking multi-drug therapy. In contrast, those who did not receive multi-drug therapy had good subjective health rates, and those who did not have arthritis showed the lowest fall experience rate (5.5%). Conclusion: The elderly population has a high prevalence of chronic diseases, and the number of people using multi-drug therapy is increasing accordingly. For this reason, healthcare providers should carefully prescribe medication for older adults. In addition, it is necessary to develop and apply a customized fall management program that reflects the demographic and health-related factors, which include diet, disease types, and health levels, with multi-drug therapy.

Comprehensive management of polypharmacy in older patients with diabetes

Background: Diabetes mellitus is a complex group of chronic metabolic conditions. The incidence of frailty, sarcopenia, geriatric syndrome, and multiple chronic conditions is high in older patients with diabetes, who often require multiple medications for optimal glucose control and treatment of associated chronic complications. Unfortunately, polypharmacy is associated with a high risk of adverse outcomes, such as hypoglycemia, drug-drug interactions, and an increased socioeconomic burden in older adults with diabetes.Current Concepts: Elderly patients with diabetes are at a high risk for polypharmacy and consequently, a high risk of adverse drug reactions. Polypharmacy is defined as the cumulative use of five or more medications. Comprehensive management and deprescribing are the most important approaches to reduce polypharmacy. Deprescription refers to reduction in or cessation of inappropriate medications.Discussion and Conclusion: Polypharmacy continues to increase the risk of morbidity and mortality among older adults with diabetes. A comprehensive polypharmacy management and deprescription plan is warranted for significant risk reduction in elderly patients with diabetes.
