Polymyalgia rheumatica and primary biliary cirrhosis.

Abstract: No significant abnormality could be seen in an electroencephalograph about two weeks after the event, but a right carotid angiograph showed narrowing of a distal branch of the middle cerebral artery in the region of the trifurcation.

“…Elevated levels of alkaline phosphatase, 1–4 5′‐nucleotidase, 2 and alanine aminotransferase, 3 prolonged prothrombin time, 3 and reduced excretion of sulfobromophthalein (Bromsulphalein,® BSP) have been reported. Varied histologic findings have been reported on liver biopsy, ranging from normal, 3 to fatty changes, 3 , 8 portal tract infiltration by lymphocytes and monocytes, 4 bile canalicular stasis, 10 primary biliary cirrhosis, 11 chronic hepatitis, 12 and finally (very infrequently), to granuloma 5–9 . This appears to be the first case of PMR associated with granulomatous hepatitis that has been reported in Israel.…”

“…The possibility that GCs can realize a confounding factor should be considered. Indeed, abnormal liver function tests typically normalize after GC therapy [ 17 ], and this can delay diagnosis of PBC. To the best of our knowledge, biopsy-proven PBC has been reported only in one patient with PMR/GCA.…”

“…Less frequent is a cytolytic pattern with elevation of alanine (ALT) and aspartate (AST) transaminases without raised AP and gamma-glutamyltranspeptidase (gGT) [16]. Some investigators hypothesized a possible immunological pathogenesis and, more specifically, reported an association with primary biliary cholangitis (PBC) [17], an autoimmune liver disease characterized by the damage of intrahepatic bile ducts. More than 95% of patients with PBC are positive for antimitochondrial antibodies (AMA).…”
