Polymerase chain reaction (PCR) for human herpesvirus 8 and heteroduplex PCR for clonality assessment in angiolymphoid hyperplasia with eosinophilia and Kimura’s disease

Abstract: We suggest that HHV-8 may not involve in a pathogenetic role in ALHE and Kimura's disease and the failure to demonstrate clonality may be consistent with the reactive nature of these diseases and lack of malignant transformation. In addition, heteroduplex-PCR can be applied to confirm doubtful cases of lymphoma in that heteroduplex-PCR is more specific than PCR as seen in our study.

“…Human herpesvirus-8 and Epstein-Barr virus could not be identified in several studies [33,34]. New etiological hypotheses have been raised by the recent description of IgG4-RD affecting the skin [11,12,13,14,15,16,17,18,19].…”

“…[15][16][17] Both diseases have J AM ACAD DERMATOL VOLUME 62, NUMBER 2 also been associated with the concurrent and or subsequent development of T-cell lymphoma; however, actual neoplastic evolution of the lymphocytic infiltrates in lesions of ALHE and Kimura disease has not been shown. [18][19][20][21][22][23][24][25] Although there have been several reports of both entities occurring in a single patient, 12,26,27 most authors view ALHE and Kimura disease as two distinct disorders. ALHE occurs in young to middleaged women, and presents with multiple small (\2 cm) erythematous dermal papules or nodules.…”

“…Molecular studies for immunoglobulin heavy-chain and T-cell receptor gene arrangement support a reactive nature. 12,13 Recently, Chim et al described a clonal T-cell receptor gamma gene rearrangement in an elderly man with Kimura disease. 14 Excision biopsy is required for a correct diagnosis and the basic histologic features are similar in different tissues which includes hyperplastic lymphoid tissue containing well-developed lymphoid follicles, infiltration of eosinophils, vascular proliferation and varying degrees of fibrosis.…”
