Background/aim: Healthcare-associated meningitis (HCAM) is a relatively rare entity with significant morbidity and mortality. The aim of this study was to systematically review the Turkish medical literature for acute nosocomial meningitis. Materials and methods:One national (ULAKBİM) and two international (www.scopus.com and www.pubmed.com) databases were searched. In addition, abstracts of four national congresses held between 2004 and 2013 were searched for reports for HCAM meningitis.Results: Data for 899 HCAM meningitis episodes were obtained from 24 reports. In terms of clinical findings, 177 of 216 (81.9%) had fever (>38 °C), 55 of 64 (85.9%) had high CRP levels, 105 of 132 had leukocytosis (>10,000/mm 3 ), and 241 of 759 had shunt infection. Cerebrospinal fluid culture yielded a pathogen in 689 of 872 nosocomial meningitis episodes. The most common pathogen was Acinetobacter spp. (30.7%), followed by coagulase-negative staphylococci (21.2%) and Staphylococcus aureus (19%). Carbapenem resistance was reported in 18 of 48 (37.5%) Acinetobacter spp. Overall mortality was 160/593 (27%). Pathogen-specific mortality was 55.5% (30/54) for A. baumannii whereas it was 18.9% (7/37) for S. aureus and 2/17 (11,7%) for MRSA. Conclusion:Nosocomial meningitis is still a serious and highly fatal disease. More preventive measures should be sought to further decrease HCAM meningitis and the mortality/morbidity related to it.

Aims: Monitoring and prevention of healthcare-associated infections (HAI) are considered to be one of the key measures to improve the effectiveness and quality of healthcare services. This study aimed to ascertain the prevalence of HAI in various hospital departments, identify the causative bacterial profile, risk factors, and the association with mortality. Methods: This prospective study included 3117 patients who were monitored in various departments of a Training and Research Hospital. The identified HAI cases were monitored using an active, prospective, rotational surveillance method. Patient data on HAI was daily recorded on pre-established tracking forms. Results: The mean hospital stay of patients was 9.9±7.5 days. The HAI prevalence was 4.5% and the HAI rate was 5.5%. The HAI rate showed no difference between internal medicine and surgical departments (5.7% vs 5.5%, p>0.05), but it was higher in intensive care units (ICU) (p<0.001). The majority of the isolated agents (65.2%) were gram-negative bacteria. Advanced age, intrinsic risk factors such as malignancy, and invasive procedures (use of central, peripheral, and urinary catheters) were associated to the development of HAI. The frequency of HAI was higher in deceased patients compared to survivors (25.4% vs 4.1%, p<0.001). Conclusion: HAI remains a major concern in hospital settings, particularly in ICU, and strongly correlates with intrinsic risk factors and invasive procedures. Optimized infection control measures with these risk factors can make a significant contribution to improving patient outcomes.

Background: The compliance of the intensive care unit (ICU) nurses regarding infection control assessment is essential for safe and quality health care. It has been seen that the knowledge of ICU nurses about nosocomial infections (NIs) is not sufficient, and they do not apply standard-contamination precautions. Objectives: This study aimed to examine the knowledge and behaviors of intensive care nurses about NIs and related factors. Methods: This descriptive-observational research was carried out in the surgical intensive care units of three hospitals in Turkey. A total of 103 eligible nurses were included in the study. An 80% classification system was used to evaluate the data. Results: The mean score of nurses' knowledge about NIs prevention was 41.52 ± 2.86. The participants' mean knowledge scores on preventing ventilator-associated pneumonia, catheter-related bloodstream infection, catheter-related urinary tract infection, and surgical site infection were 15.86 ± 1.54, 13.08 ± 1.54, 5.98 ± 0.83, and 6.59 ± 0.95, respectively. The mean behavior score for NIs prevention was found as 15.86 ± 1.54. The participants' mean scores on prevention of ventilator-associated pneumonia, catheter-related bloodstream infection, catheter-related urinary tract infection, and surgical site infection were 6.93 ± 0.89, 4.77 ± 0.82, 4.67 ± 0.71, and 1.10 ± 0.31, respectively. The mean scores of female nurses' knowledge on NIs prevention were higher than males (P < 0.05). A statistically significant difference was found between the nurses' weekly working hours and their knowledge and behavior scores regarding NIs prevention (P < 0.05). Conclusions: It was determined that the knowledge and behaviors of intensive care nurses regarding the prevention of NIs were insufficient. Also, gender and weekly working hours contribute to knowledge and behaviors related to NIs prevention.
