Point of Care Testing in Cardiac Surgery: Diagnostic Modalities to Assess Coagulation and Platelet Function

Abstract: Point of Care testing in the perioperative arena is a high priority topic, specifically in cardiac surgery where a majority of perioperative blood transfusions occur. Platelet function in particular is of interest in cardiac surgery as cardiopulmonary bypass itself creates known platelet dysfunction, but also because more and more patients are on antiplatelet agents due to coronary stenting procedures. This review will focus on presenting current data relating to platelet function in the perioperative setting,… Show more

“…This level is considered a cut-off for strong inhibition of COX-1 by aspirin [ 22 ]. The ASPItest is confirmed to be a reliable tool in aspirin resistance diagnosis [ 23 26 ]. A high frequency (34%) of aspirin resistance in heart transplantation recipients was found in the present study.…”

“…Furthermore, Sonoclot allows rapid identification of platelet dysfunctions, which are associated with increased mortality in patients with coagulopathy [16,17]. Recently, results obtained from animal experiments and in our large patient cohort have suggested that alterations in hemostasis might be detected by Sonoclot and predicted coagulation dysfunction in patients with overt DIC.…”

“…Thromboelastography (TEG; ROTEM, TEM Innovations, Munich, Germany) is used to assess clot formation, including the kinetics of clotting, clot strength, and lysis that involves globally platelet function and coagulation [Scharbert et al., 2009 ; Stafford and Weitzel, 2013 ]. This test has been traditionally used in the surgical setting as a screen for bleeding risk.…”
