PO2 21% oxygenated hypothermic machine perfusion in kidney transplantation: Any clinical benefit?

Abstract: Background: In deceased donor kidney transplantation (KT), the use of hypothermic machine perfusion (HMP) has been acquiring the status of best practice in the pre-transplant management of kidney grafts. Two types of HMP are currently available, oxygenated HMP and non-oxygenated HMP. However, data on the real clinical impact of oxygenation on KT outcome are still heterogeneous. Methods: Retrospective study on a cohort of 103 patients transplanted with a single kidney graft that was managed either with end-isch… Show more

“…Authors describe less inflammation and better endo-and exocrine pancreas function during later evaluation on the normothermic perfusion device (Tables 2,3) [40]. Similar results are known from kidneys and livers; perfusate oxygen levels of 21% or a pO2 of <20kPa (8-18kPa) is not enough to trigger metabolic changes and rebuild ATP [60][61][62][63][64]. Subsequently, HMP without active oxygenation as done with the LifePort for many years, has limited effects.…”

“…These positive findings could not be confirmed in two matched-case studies. Different studies assessing the effects of end-ischemic HMPO 2 in ECD DBD grafts could not demonstrate an improvement in DGF and PNF rate nor early graft function and biopsy-proven acute rejection rate as compared to SCS alone [63,66,[78][79][80].…”

“…Currently, six clinical trials studied the effect of active oxygenation during HMP with a variety of O 2 administration strategies, as detailed in Table 1 [63,65,66,[78][79][80]. The strongest evidence for active oxygenation during HMP is derived from the multicenter randomized controlled trial (RCT) including 50 years or older DCD Maastricht category III donors.…”
