Pneumothorax in patients with respiratory failure in ICU

Abstract: in medical management of ICU patients over time (6). Pneumothorax can be misdiagnosed at initial presentation in about 32% of patients admitted to the ICU (7). It has a mortality rate of 68%, with the worst prognosis being in patients with septic shock and barotrauma (3). Etiology and risk factorsAir can enter the pleural space by three routes: (I) communication between alveoli and pleura, (II) communication (direct or indirect) between atmosphere and pleural space, and (III) presence of gas producing organism… Show more

“…The total percentage of IMV-related air leakages in the study hospital in the past 5 years was 7.5%, which is consistent with the literature. In addition, this study concluded that air leakages increased the length of hospitalization stay and the duration of mechanical ventilation in patients, in agreement with previous reports of adult patients [ 15 , 16 ].…”

“…Before putting the patient on a mechanical ventilator, we should rule out pneumothorax, as mechanical ventilation can exacerbate pneumothorax. Our patient on admission had bilaterally diminished breath sounds and a hyper-resonant note suggesting he had bilateral pneumothorax prior to mechanical ventilation [7].…”

“…Patients with particularly complex prolonged ICU courses were also at increased risk for longterm ventilation, indicative of the use of Extracorporeal lung assist (ECLA) or extracorporeal membrane oxygenation (ECMO), positioning therapy, transfusion of plasma components or coagulation factors. The use of a chest tube as a further risk factor indicates either a pre-existing pulmonary condition or complications related to barotrauma or iatrogenesis [23]. In addition, procedures suggestive of leading neurological problems such as cerebral spinal surgeries, cerebrospinal uid system surgeries, or cranial imaging were also predictors for unfavourable outcomes.…”
