Pneumothorax in Patients with Pulmonary Langerhans Cell Histiocytosis

Abstract: IntroductionPneumothorax often develops in pulmonary Langerhans cell histiocytosis (PLCH), but some patients take a long time to be correctly diagnosed.ObjectivesThis study assessed the frequency of pneumothorax in PLCH and analysed the role of chest computed tomography (CT) in the prompt diagnosis.Patients and materialOf the 90 patients with PLCH seen from 2000 to 2015, 29 (32%) had pneumothorax as the initial finding. In this group, 18 (62%) patients were diagnosed within 1 month, whereas the diagnosis was d… Show more

“…The 50% rate of pneumothorax recurrence and the median number of 2 pneumothorax episodes in our study population were somewhat similar to those reported in a previous smaller series of 16 patients [3] and higher than that observed in a more recent larger study [2]. The rate of pneumothorax recurrences after PLCH is significantly lower than what was reported in LAM and BHD syndrome [810].…”

“…Pneumothorax occurs in approximately 15–20% of PLCH patients [1]. It may be bilateral, recur during the course of the disease, and its management is not standardised [2]. The only one available series of 16 patients reported a high rate of pneumothorax recurrence (58%) after conservative treatment including chest tube drainage, as compared to no recurrence after thoracotomy (TCT) [3].…”

“…Pneumothoraces are less common in children compared to the adult population [ 8 , 18 , 19 ]. R adzikowska et al [ 21 ] found that 32% of their predominantly adult population group had pneumothoraces at initial presentation.…”

“…The diffusing capacity could be impaired by itself or accompanied by restrictive, obstructive or mixed ventilation abnormalities, which was in agreement with previous studies [ 4 , 7 ]. Radzikowska et al presented that patients with pneumothorax had worse FEV, FEV1 and TLC than those without pneumothorax in a cohort involving 90 patients [ 20 ]. We also found that patients with a history of pneumothorax had worse DLCO, FEV1 and FEV1/FVC.…”
