Hyperpneumatization of the paranasal sinuses is a rare condition. Hypersinus, pneumosinus dilatans, and pneumocele are the three types of excessive pneumatization according to the traditional classification proposed by Urken et al. (1987). If in the first case, no treatment is required, but the latter two may be accompanied by aesthetic and/or functional disorders or may result in pressure effects on adjacent anatomical objects, therefore they require surgical correction. Maxillary sinus pneumocele is described in few publications. The presented case demonstrates typical clinical and radiographic signs of the maxillary sinus pneumocele, as well as hypersinus on the contralateral side. Cone beam computed tomography scans in combination with anamnestic and clinical data provide key information for the diagnosis of maxillary sinus pneumocele, as well as for differential diagnosis with other variants of their excessive pneumatization.

Introduction Sinus pneumatization by definition is a cystic enlargement within the paranasal sinuses, and that expansion causes clinical symptoms. Importance This case highlights the importance of identifying clinically significant sinus pneumatization and the significant role of surgical treatment. Case presentation This case report describes a clinical presentation of intramaxillary sinus cell affecting the patient's quality of life. Despite the rare location of the cyst, it was diagnosed by computed tomography and managed by Functional Endoscopic Sinus surgery (FESS). Clinical discussion This case discussion describes the nomenclature, classifications, location and the treatment of sinus pneumatic dilatations. Conclusion Our case demonstrates the importance of managing the underlying cause of the patient symptoms by a thorough history and physical examination and it can be confirmed by imaging. Symptomatic maxillary air cysts, albeit controversial in its nomenclature and mechanism, should be addressed surgically as it carries high levels of success.

Pneumosinus dilatans multiplex associated with hormonal imbalance

Pneumosinus dilatans describes an abnormal dilation of one or more paranasal sinuses without radiological evidence of localised bone destruction, hyperostosis or mucous membrane thickening. Dilation of mastoid air cells also occurs rarely along with involvement of paranasal sinuses. This rare combination of unknown aetiology was reported in two cases in the literature and termed ‘Pneumosinus Dilatans Multiplex’ (PSDM). It is usually asymptomatic, and is detected incidentally on plain radiography, CT or MRI. If left untreated, it can further erode the bone leading to complications such as facial asymmetry, neurological disorders and pathological fractures. The aetiology of the condition remains obscure. Various hypotheses proposed are the presence of gas-forming microorganisms, spontaneous drainage of a mucocele, the presence of a one-way valve, dysregulation of hormonal levels leading to a disturbance of osteoblastic and osteoclastic activity. This paper describes a case of PSDM possibly secondary to hormonal disturbance.

Abstract: Whilst the nomenclature of this pathology is confusing, distinctions of terminology do not alter the management. Hence, we support the use of the term 'air cyst', to incorporate all these lesions.
