Pneumomediastinum is a frequent but minor complication during esophageal endoscopic submucosal dissection

Abstract: In esophageal ESD, pneumomediastinum detected by chest CT only does not cause clinically significant complication. Endoscopic muscle exposure during ESD is a significant risk factor for pneumomediastinum, which causes mild inflammation in the early post-ESD phase.

“…The en bloc resection rate is greater than 90% (90 .6%-100%) [24,29,[45][46][47][48][49] . En bloc resection, meaning resection in a single piece, facilitates an accurate histological assessment and reduces the risk of recurrence.…”

“…En bloc resection, meaning resection in a single piece, facilitates an accurate histological assessment and reduces the risk of recurrence. In fact, the local recurrence rate after esophageal ESD is extremely low (0%-3.1%) [24,29,[45][46][47][48][49] . In contrast, the local recurrence of SCCs after EMR was reported to be as high as 20% because en bloc resection by EMR is difficult and multiple resections are required for large lesions [17] .…”

“…Other less frequent complications described in ESD are aspiration pneumonia, transient bacteriemia (112), phlegmonous gastritis (130), gastric ischemia, gastric hematoma (131), transmural pneumatosis, deep vein thrombosis, mediastinal emphysema and tension pneumomediastinum or pneumoperitoneum (132)(133)(134)(135). Peritoneal carcinomatosis is a rare complication and only one case has been published after gastric perforation.…”

“…The overall incidence of bleeding and perforation with ESD is 11% and 6% respectively [32][33][34]. Due to the widespread acceptance of gastric and oesophageal ESDs, the number of medical facilities that perform colorectal ESDs grew in recent years [35,36].…”
