Pneumatocele formation in a patient with Proteus mirabilis pneumonia

Abstract: Summary:A rare case of pulmonary pneumatoceles complicating the course of Proteus pneumonia in an adult is described. The pneumatoceles which appeared early in the course of the pneumonia resolved completely without complications. A brief review of the literature regarding formation of cavities in Proteus pneumonia is given.

“…The high frequency of P. mirabilis recovery from chicken livers (Table 2) is alarming, due to the documented literature of the involvement of this organism in different human conditions including pneumonia, 8,10 septicemia, central nervous system infection, 11 food poisoning, 32,33 and urolithiasis. 12 In addition, P. mirabilis is a proteolytic bacterium that is proven lately to alter the pathogenicity of avian influenza viruses, due to its ability to cleave the hemagglutinin protein of this virus.…”

“…[1][2][3][4][5] The most important poultry organisms causing zoonoses are Salmonella spp., 5 Campylobacter spp., 6 and avian influenza. 3,4 Proteus mirabilis has been reported as one of the causative agents of human pneumonia 7,8 and other lung infection conditions. 9 Proteus mirabilis causes 90% of Proteus infections and is mostly found in people with compromised immune system.…”

“…The occurrences of P. mirabilis pneumonia were sporadic and showed no evidence of outbreak in the hospital. In the literature, community-acquired pneumonia is reported in only one female patient aged 47 years who developed a pneumatocele [3], and the frequency of P. mirabilis becoming the causative organism of community-acquired pneumonia was considered as extremely low.…”

“…In such instances, emergent decompression is indicated. There have been several case reports of percutaneous decompression of tension PC by needle aspiration, 11 catheter drainage, 4,32 or chest tube drainage 12 under CT or fluoroscopic guidance. Persistent air leak from bronchopleural fistula formation during mechanical ventilation is a serious complication of ventilator therapy leading to inefficient ventilation through increased dead space ventilation.…”
