The objective of this study was to evaluate the efficacy and safety of super pulse CO2 laser-assisted punctoplasty with canalicular curettage in primary canaliculitis. In this retrospective serial case study, the clinical data of 26 patients who underwent super pulse CO2 laser-assisted punctoplasty for the treatment of canaliculitis were collected from January 2020 to May 2022. The clinical presentation, intraoperative and microbiologic findings, surgical pain severity, postoperative outcome, and complications were studied. Of the 26 patients, most were females (female:male 20:6), with a mean age of 60.1 ± 16.1 years (range, 19–93). Mucopurulent discharge (96.2%), eyelid redness and swelling (53.8%), and epiphora (38.5%) were the most common presentations. During the surgery, concretions were present in 73.1% (19/26) of the patients. The surgical pain severity scores ranged from 1 to 5, according to the visual analog scale, with a mean score of 3.2 ± 0.8. This procedure resulted in complete resolution in 22 (84.6%) patients and significant improvement in 2 (7.7%) patients, and 2 (7.7%) patients required additional lacrimal surgery with a mean follow-up time of 10.9 ± 3.7 months. The surgical procedure of super pulse CO2 laser-assisted punctoplasty followed by curettage appears to be a safe, effective, minimally invasive, and well-tolerated treatment for primary canaliculitis.

Kanalikulitis terjadi akibat infeksi pada kanalikulus sistem drainase lakrimal, yang menyebabkan penonjolan eritematosa pada punctum lakrimal dan eritema pada kulit di sekitarnya, kadang disertai sekret dan/atau konkresi mukopurulen. Paling sering, bakteri patogen terlibat, meskipun kanalikulitis juga dapat berasal dari infeksi jamur atau virus. Pada individu yang lebih tua, Actinomyces israelii (bakteri anaerob) adalah etiologi yang paling umum. Divertikulum atau obstruksi kanalikuli dapat meningkatkan pertumbuhan mikroorganisme. Kondisi ini merupaknn bentuk yang tidak biasa dan sering salah didiagnosis oleh dokter mata. Kondisi tersebut memerlukan kecermatan dalam melakukan diagnosis. Katarak adalah kekeruhan atau kekeruhan lensa mata yang biasanya jernih atau kapsulnya (selaput transparan yang mengelilinginya) yang mengaburkan jalannya cahaya melalui lensa ke retina mata. Penyakit yang penurunan penglihata ini dapat menyerang bayi, orang dewasa, dan orang tua. Katarak merupakan penyebab signifikan kebutaan di seluruh dunia. Pilihan pengobatan termasuk koreksi dengan kacamata namun hanya pada tahap awal, dan jika katarak cukup matang untuk mengganggu aktivitas rutin, operasi mungkin disarankan.

In Situ Forming Hydrogel as a Tracer and Degradable Lacrimal Plug for Dry Eye Treatment

Lacrimal plug is an effective and widely therapeutic strategy to treat dry eye. However, almost all commercialized plugs are fixed in a certain design and associated with many complications, such as spontaneous plug extrusion, epiphora, and granuloma and cannot be traced in the long‐term. Herein, a simple in situ forming hydrogel is developed as a tracer and degradable lacrimal plug to achieve the best match with the irregular lacrimal passages. In this strategy, methacrylate‐modified silk fibroin (SFMA) is served as a network, and a self‐assembled indocyanine green fluorescence tracer nanoparticle (FTN) is embedded as an indicator to develop the hydrogel plug using visible photo‐crosslinking. This SFMA/FTN hydrogel plug has excellent biocompatibility and biodegradability, which can be noninvasively monitored by near‐infrared light. In vivo tests based on dry eye rabbits show that the SFMA/FTN hydrogel plug can completely block the lacrimal passages and greatly improve the various clinical indicators of dry eye. These results demonstrate that the SFMA/FTN hydrogel is suitable as an injectable and degradable lacrimal plug with a long‐term tracking function. The work offers a new approach to the development of absorbable plugs for the treatment of dry eye.
