Pleurodesis

Abstract: Symptomatic pleural disease, specifically malignant pleural effusion, refractory benign pleural effusion, and pneumothoraces are common diseases that often require therapeutic interventions. The spectrum of management strategies often includes selection of a chemical pleurodesis agent administered in combination with an indwelling pleural catheter or chest tube.Additionally, there is a role for minimally invasive techniques which include medical thoracoscopy or more advanced video-assisted thoracoscopic approa… Show more

“…Differences in the efficacy of pleurodesis between silver nitrate and talc could partially explain the observed discrepancy in the results observed between 7 and 28 days in the two pleurodesis agent groups in the study by Teixeira and colleagues 45 . Silver nitrate has been associated with better efficacy (96% vs 84%) in achieving pleurodesis, while it has also been used as a backup agent after talc failure 68‐70 …”

“…Findings from lymphangiography can help guide surgical options and may even be therapeutic in patients with low-volume lymphatic leakage [ 9 ]. Pleurodesis is another option for patients, though it is typically a salvage treatment for refractory chylothorax in non-surgical candidates [ 9 - 10 ]. It aims to create adhesions between the parietal and visceral pleura to prevent the accumulation of fluid in the space [ 10 ].…”

“…As sclerosant agents, tetracycline, talc, iodopovidone are used to achieve chemical pleurodesis by triggering an inflammatory response [17,18]. According to Rice et al and Mc-Carthy et al, local tetracycline application had no effect on seroma formation after mastectomy and was associated with severe pain [19,20].…”
