Pleomorphic adenoma palate: Major tumor in a minor gland

Abstract: Pleomorphic adenoma (PA) is the most common benign mixed salivary gland neoplasm that accounts for 60% of all benign salivary gland tumors. It has diverse histological presentation and occurs in both major and minor salivary glands. PA of minor salivary gland in the palate is a common entity. We report the case of a 45-year-old female who presented with a painless slow growing swelling of palate over the last 20 years. The mass was extending to oropharynx causing mechanical obstruction of airway. Magnetic reso… Show more

“…The recommended treatment for pleomorphic adenoma in minor salivary glands is wide local excision, which involves the removal of the tumor along with the periosteum or bone if they are involved. Radiation therapy is not advised as these tumors are resistant to its effects [19]. Simple enucleation of the tumor is discouraged as it is associated with a high rate of local recurrence and should be avoided [1,10].…”

“…The treatment of PA is essentially surgical excision [7-8]. Because these tumors are radioresistant, radiation therapy is contraindicated [9]. Though these benign tumors are well-encapsulated, resection of the tumor with an adequate margin of grossly normal surrounding tissue is necessary to prevent local recurrence because these tumors are known to have microscopic pseudopod-like extensions into the surrounding tissue due to "dehiscence" in the capsule [10].…”

“…At 9 × 7.5 × 3.9 cm, the PA in our case represents the largest soft palate PA reported in the literature. The previous largest soft palate PA was 7 × 6 cm [ 6 ], with multiple various midsized tumours also being reported [ 7 ].…”
