Platelet Serotonin as Biomarker for Assessing Suicidal Behaviour in Patients with Bipolar I Disorder

Abstract: Context. Suicide is a global public health issue. Bipolar disorder (BPD) has the highest suicide risk among individuals suffering from mental disorders. Serotoninergic dysfunctions have been linked to suicidal behaviour and platelet serotonin is recognised as a reliable index for the presynaptic serotonin activity.Objective. Our aim was to assess whether alterations occur in platelet serotonin concentrations in BPD type I in respect to suicide attempters compared with nonattempters.Design. This was a cross-sec… Show more

“…Our results are in line with current research findings that support higher cortisol levels in individuals with suicidal ideation (35,36) and submit to the allostatic hypothesis age-dependent maladaptive responses of the HPA axis to stress exposure overtime (28) and with recent scientific literature which correlates high suicide risk with a hyperactive HPA axis in younger individuals, that later downregulates with age, after chronic exposure (21).…”

“…These findings are in line with the allostatic load theory that postulates agedependent maladaptive responses of the HPA axis to stress exposure overtime (28) and with recent scientific literature which correlates high suicide risk with a hyperactive HPA axis in younger individuals, that later down-regulates with age, after chronic exposure (21). Results of the association between suicidal behaviour, platelet serotonin (35), or cortisol levels were also highlighted in other research studies (36). Even though our research did not reveal any significant correlations between suicide attempts and cortisol levels in BPD I, the data shows trends that reflect a relationship that should be further explored with larger samples size.…”

“…In turn, the aggressivity and impulsivity of suicidal attempters seem associated with serotonin content and the total number of 5-HT 2A receptors on platelet surface [ 39 ]. A recent study showed significantly lower levels of platelet serotonin in suicide attempters compared to non-attempters and high- vs. low-lethality attempters, respectively [ 40 ]. Therefore, a strict interplay between platelet levels, serotonin, psychopathological features of (hypo)mania, and suicide in patients with BD could be hypothesized.…”

“…Furthermore, it was recently established that low serotonin levels in plasma were associated with patients with a low response to antidepressant treatment, presenting depressive symptoms (Holck et al, 2019). Additionally, platelets with low serotonin levels are a reliable biomarker in suicide risk assessment (Giurgiuca et al, 2016). Thus, lower levels of serotonin in plasma suggest an increased presence of depressive symptoms according to the CESD-20 score.…”
